SHINING LIGHT ON ORAL CANCER

DENTAL CC 28

A comprehensive dental exam includes looking for signs of oral cancer which has been increasingly occurring in young adults under age 40 during the past three decades. Oral cancer is also linked to a high mortality rate, but early diagnosis can lead to a five-year survival rate as high as 81%. With all this in mind, dentists have enlisted the help of an instrument known as the Velscope to help spot early signs of cancer in the soft tissues of the mouth. This screening device emits a safe, high-energy blue light that helps the dentist identify abnormalities that are not readily apparent to the unassisted eye. The examination takes only minutes, but its benefits can help extend lives for years.

P.S. Tobacco use accounts for most oral cancers, while alcohol use is another contributing factor.

FULL (DENTURE) DISCLOSURE

DENTAL CC 27

Patients fitted with full dentures may mistakenly believe that they no longer must worry about scheduling comprehensive oral exams. After all, there are no natural teeth left to examine. However, the mouth and bite change constantly, which means that the dentist must regularly assess denture fit. If that is the case, the dentist will reline full dentures that do not fit as well as they should. Patients should also note that any man-made material will eventually show signs of wear. This is particularly true of plastic that is subjected to the considerable forces exerted by the jaws while biting and chewing. Gum tissue underlying dentures must also be regularly examined for signs of irritation and pre-cancerous lesions.

At JOLLY SMILES, we can help you decide on the right option for your situation. If you have questions about your dentures, we invite you to call our office. We want to provide you and your family with the best dental care possible in a comfortable, professional environment. Prevention - brushing, flossing, and regular professional cleanings and checkups - is still the best road to dental health. Our office is located at 102 Sleepy Hollow Drive, Suite 100. Please call 302-378-3384 to schedule an appointment. Evening appointments are available. We use Sapphire whitening. Most insurance plans accepted.

P.S. If a patient is unable to successfully wear full dentures, the dentist may recommend implants or anchoring an overdenture to existing tooth roots.

A LUSTROUS-LOOKING SMILE

DENTAL CC 26

Patients with discolored front teeth that cannot be corrected with tooth-whitening procedures may resort to porcelain veneers. The application of these fingernail-thin shells over the front of the teeth provide a natural-looking solution for teeth that are dull, worn, chipped, cracked, or unevenly spaced. The procedure involves removing only enough enamel from the affected tooth’s surface to accept the veneer. Aside from selecting a color for the veneers that will match natural tooth color, dentist and patient may agree to revise the smile by choosing a suitable tooth shape. The end result is a durable restoration that displays the translucency of a real tooth. As light reflects off the tooth, it gives off a lustrous, enamel-like appearance

P.S. While dental veneers, composed of composite resin material, may not be as translucent as porcelain veneers, they are a good-looking, less-expensive alternative.

SWEET!

DENTAL CC 25

Those concerned about both their overall health and oral health should take note of the benefits of the naturally occurring sugar substitute known as “xylitol.” First discovered in the late 19th century, and popularized as a safe sweetener for diabetics that would not impact insulin levels, xylitol was later found to have significant dental benefits as well. This substance, which is found in a variety of fruits and vegetables, is just about as sweet as sucrose (ordinary sugar) but has fewer calories and no aftertaste. More importantly, xylitol prevents decay-causing bacteria from sticking to teeth, thereby reducing plaque formation. Research also shows that xylitol helps repair damaged enamel by stimulating saliva production. Look for xylitol in sugarless gum

TREATING MOM’S GUMS SAFE FOR BABY

DENTAL CC 24

As research continues to uncover important links between the health of the gums and overall health, it is becoming increasingly clear that gum inflammation (gingivitis) should be prevented or treated early. This holds especially true for pregnant women, who undergo hormonal changes that render them more susceptible to gum disease. While standard antibiotic-based therapy must be avoided because it stains babies’ teeth, aggressive tooth-cleaning (“scaling and planing”) is also often avoided in the belief that it could lead to bacteria entering the blood stream and harming the fetus. However, recent research finds this fear to be unfounded. Testing of toddlers whose mothers were treated with aggressive cleaning while pregnant showed no signs of developmental harm. Prevention is always preferred, however.

P.S. Studies indicate that periodontal (gum) treatment significantly reduces the risk of preterm birth or low birth-weight infants.

SMILE MAKEOVER

DENTAL CC 23

Because your smile is one of the first things that people notice about you, it makes sense to have your teeth, gums, and lips carefully assessed by a dentist. To begin with, tooth-whitening can improve the color of yellowed and stained teeth, and replacement of amalgam (silver) fillings with tooth-colored composite materials can further whiten smiles. If necessary, composite bonding or porcelain veneers can be used to cover damaged teeth, while bridges, crowns, or implants fill in the gaps left by missing teeth. Irregularities in tooth length and shape can be addressed with tooth-contouring procedures so that the smile line formed by the edges of the upper teeth follows the curvature of the lower lip. These changes get noticed!

TOOTH-CLENCHING NEWS

DENTAL CC 22

Daily bombardment with news of financial meltdowns, foreclosures, and natural/environmental disasters seems to have a significant number of people literally clenching their teeth. It may be glib to say that such news is all part of the “daily grind,” but these stress-inducing events are no laughing matter for those suffering from “bruxism.” This term refers to the grinding and clenching of teeth that many stressed-out individuals endure while they sleep. According to the Academy of General Dentistry, about one in three people suffers from bruxism, particularly aggressive, competitive, and hurried individuals. As their anxiety mounts, teeth and prosthetic crowns become more susceptible to fracture. To protect their teeth, bruxers are encouraged to see the dentist about wearing protective night guards.

P.S. Bruxism is associated with temporomandibular joint (TMJ) disorder and sleep apnea.

TEETHING REMEDY WARNING

DENTAL CC 21

The FDA recently issued a safety announcement that warns parents against using benzocaine teething products in children under the age of two years. The use of these products (including Orajel, Baby Orajel, Orabase, and Anbesol) can lead to a serious condition known as “methemoglobinemia,” which involves the inability of the oxygen-carrying protein in red blood cells to transport oxygen to the body’s cells and tissues. Fortunately, there are a number of alternative treatments to benzocaine teething gels, including chilled rubber rings that babies can chew on to soothe their teething discomfort. Solid frozen rings, on the other hand, should be avoided since they can cause injury to baby’s tender gums. Parents can also massage their baby’s gums with clean fingertips

P.S. A good teething remedy is a wet washcloth that is twisted into a rope and set in the freezer for a few minutes. Teething babies can chew on the washcloth until it returns to room temperature.

DOES PERIODONTITIS INCREASE CANCER RISK?

DENTAL CC 20

As evidence mounts linking gum disease to other serious conditions such as heart disease and even premature birth among pregnant women, dentists and hygienists have been highlighting the importance of maintaining the health of the gums. Not only is it critical to brush and floss twice daily at home, patients are strongly encouraged to undergo professional dental cleanings to prevent or alleviate inflammation of the gums (“gingivitis”). Otherwise, if left untreated, gum disease may lead to a more serious condition, chronic periodontitis. This advanced form of gum disease can result in bone and tooth loss. In addition, research indicates that periodontitis also increases the risk of developing head and mouth cancers, especially in the mouth and throat.

P.S. According to the research mentioned above, each millimeter loss of alveolar bone (the ridge of bone that surrounds the roots of the teeth) increases the risk of head and neck cancer fourfold.

CHOOSING SUITABLE REPLACEMENTS

DENTAL CC 19

While fluoridated drinking water has helped ensure that people under age 50 have an excellent chance of keeping all their teeth, older individuals may not be so fortunate. In fact, the Centers for Disease Control and Prevention reports that one-quarter of American adults over the age of 65 have lost all their teeth; the National Institute of Dental and Craniofacial Research indicates that 58% of Americans over age 50 have fewer than 21 of the normal 32 teeth. Missing teeth lead to poor nutrition as well as a possible collapse of the vertical dimension of the lower face. To avert these potential problems, dental implants provide the best functioning, and most natural-looking, replacements.

July 4, 2011 column dental CC-19

If you have any questions about your chances for replacing lost teeth through the latest techniques call JOLLY SMILES at 302-378-3384. Our recommendations for dental treatment are always in the best interest of the patient. We can provide you with implants, dentures, bridges and crowns, as well as general dental treatment for the entire family. We want you to look and feel your best. We offer Sapphire whitening. Most insurance plans accepted.

P.S. Dental implants are rooted inside the jawbone, which prevents the bone resorption that causes the bone underlying lost teeth to shrink.

FOODS FOR THOUGHT

DENTAL CC 18

While we have all been told that eating too much candy is bad for our teeth, relatively few patients are aware that seemingly healthy and nutritious foods such as raisins and whole wheat bread also pose potential problems. The fact is that all carbohydrates break down into simple sugars, which then are converted into “plaque” by bacteria in the mouth. It is this sticky residue that is primarily responsible for tooth decay and gum disease. This risk is increased when carbohydrate-based foods such as raisins and whole-grain breads adhere to tooth surfaces and collect below the gum line. At this point, they can pose nearly as much of a danger to oral health as caramel in candy bars

P.S. To protect your teeth from the acid in sodas, sip water or chew sugarless gum to activate acid-neutralizing saliva, and then brush your teeth.

ROOT CAUSE OF PAIN

DENTAL CC 17

When a deep cavity or crack causes infection that damages a tooth’s pulp (which contains blood vessels, nerves, and connective tissue), severe pain results. If left untreated, the damage can spread to the surrounding bone, causing further problems. For this reason alone, patients with an infection in the pulp of a tooth need to undergo endodontic treatment that involves drilling to the core of the tooth; removing the damaged pulp; and then cleaning, shaping, filling, and sealing the tooth. A “root canal” procedure derives its name from the fact that the pulp branches down each root through canals that stop just short of the bottom tip of the tooth. Endodontic treatment preserves teeth and circumvents the need to extract them.

June 20, 2011 column dental CC-17

Our mission is to use our knowledge and experience in endodontics to save teeth that would otherwise be lost. At JOLLY SMILES, we are currently accepting new patients. We provide individual care to our patients in a comfortable, professional environment. If you know of anyone in need of quality dental care we would appreciate your referrals. Please call 302-378-3384 to schedule an appointment. Evening appointments are available. Most insurance plans accepted.

P.S. Root canal treatment removes the source of pain caused by an infected pulp; because the procedure is performed with local anesthetic, it does not cause pain.

ALL-PORCELAIN CROWNS

DENTAL CC 16

When it comes to choice of material for replacement crowns, porcelain provides the most natural appearance. Much like natural teeth, translucent porcelain does not have a monochromatic appearance. Instead, it displays color variation that results from the internal play of light. Porcelain’s texture also plays a role in imparting an organic look to replacement teeth by breaking up light reflections. The first all-ceramic crowns introduced over a century ago were the porcelain-jacket type. While they offered superior cosmetic appearance, they were not thought to be strong enough to be used on back (chewing) teeth. More recently, however, proprietary materials and processes have been developed for fabricating all-ceramic crowns out of solid blocks of material that combine excellent strength and beauty.

P.S. All-ceramic crowns tend to be more natural looking than porcelain-fused-to-metal (PFM) dental crowns, and they give no indication of the metal edge of their substructure like PFM crowns do.

PLACE-SETTING TEETH

DENTAL CC 15

“Baby” (or deciduous) teeth may begin to fall out and be replaced by permanent teeth starting at approximately six years of age, but that does not mean that they are expendable. Baby teeth play an important role in aligning the spacing of subsequent permanent teeth. They also serve to support the upper and lower jaws in proper alignment. Thus, parents will want to join the dentist in doing everything possible to preserve the health of their children’s baby teeth. If these place-setters are lost prematurely to injury or decay, it may adversely affect the positioning of the teeth that will replace them. For this reason and others, children should adopt healthy oral care habits at an early age.

P.S. As emerging permanent teeth push against the roots of overlying deciduous teeth, the roots of the baby teeth dissolve and become absorbed by the forming permanent teeth.

CHLORINATED POOL WATER’S POTENTIAL RISK

DENTAL CC 14

There is little question that swimming is a healthy activity. However, those who frequently swim or spend prolonged periods of time in chlorinated pools should be aware of the potential for tooth enamel damage. While chlorine is added to pool water to provide swimmers protection against bacteria, using too much chlorine can lower pH levels in water to the point where it can erode tooth enamel. Curiously, brushing teeth after they are exposed to chlorinated pool water with low pH (acidic) may do more harm than good. Abrading the weakened tooth enamel with a brush may only hasten the erosion. Rinsing with a fluoride product may be preferable. Consultation with the dentist can provide a proper diagnosis and treatment.

P.S. Because most sodas and sport drinks contain acids, it may benefit teeth to drink them through a straw.

PLAQUE DETECTION

DENTAL CC 13

Although your teeth may seem clean, they still may be encrusted with germ-laden plaque, especially between teeth and at the gum line. To detect just how effective tooth brushing and flossing are in removing the sticky substance that causes both tooth decay and gum disease, patients can undergo a plaque detection test either at home or in the dental office. The home version of the test utilizes special tablets that contain red dye. Once the tablet is chewed and mixed with saliva, the dye is swished around in the mouth for about 30 seconds. After a water rinse, the teeth are examined. Any pink residue on the teeth indicates remaining plaque, which poses a threat to teeth and gums.

MISSING LINK UNCOVERED

DENTAL CC 12

Proline is one of a number of amino acids that serve both as “building blocks” of proteins and as intermediates in metabolism. What has recently come to the attention of dentists is the fact that, in addition to amino acids’ role as the building blocks of muscles, tendons, organs, glands, nails, and hair, proline has been identified as the amino acid responsible for making teeth strong and resilient. Researchers have found proline to be repeated at the center of proteins found in tooth enamel, which is the hardest substance found in the human body. This new finding may lay a foundation for future research that helps dentists replace damaged parts of the outer-tooth layer with healthy layers of new enamel.

P.S. Other animals, such as frogs, do not have proline repeats as long as those found in humans’ teeth, which causes frogs’ teeth to be softer than those of humans.

TOOTH RESHAPING

DENTAL CC 11

When minor defects such as small chips, shallow pits, and worn edges detract from a smile, “dental contouring” (or “tooth reshaping”) provides a quick, safe, and easy fix. This cosmetic procedure may also help correct crooked and overlapping teeth and may even be a substitute for braces under certain circumstances. The procedure involves using a sanding drill to artfully and gradually remove minute amounts of tooth enamel in order to attain the desired shape or eliminate any unwanted characteristic. The dentist will likely mark the teeth with a pencil prior to the procedure to outline the work. As the teeth are sculpted, imperfections are eliminated or minimized. Finally, the teeth are smoothed and polished. Anesthetic is rarely needed.

PIERCING INTELLIGENCE

DENTAL CC 10

The American Dental Association opposes tongue piercing due to potential complications arising from puncturing the tongue and inserting a decorative post. Perhaps the most serious of such complications is the risk of infection, one source of which is thin layers of bacteria (called biofilms) that coat piercings and attract germs. Interestingly, a recent study found that stainless steel and titanium studs are likely to collect more bacteria than plastic studs. An examination of pierced tongues showed that stainless steel studs harbored the most bacteria (including Staphylococcus aureus and Haemophilus influenza, which can cause body-wide infections). The titanium studs collected bacteria to a lesser extent, while plastic studs accumulated significantly fewer bacteria. However, any tongue piercing poses an unnecessary risk.

P.S. Other complications of tongue piercing include tooth chipping, gum disease, and gum recession.

FAT CHANCE!

DENTAL CC 9

Not only does gum disease afflict a large portion of the adult population and pose the threat of tooth loss, this inflammatory condition has also been linked with an increased risk of heart disease, stroke, and other serious conditions. With this in mind, patients should be scrupulous about oral care at home and scheduling regular dental appointments. It may also pay to increase consumption of omega-3 fatty acids found in fish. New research indicates that omega-3s significantly decrease the risk of gum disease. People consuming the most DHA (one of the principal omega-3 fatty acids found in fish) were 22 percent less likely to suffer periodontitis; EPA (the other important fish-oil omega-3) also lowered the risk, but not as much.

ARE YOU COVERED?

DENTAL CC

Standard dental insurance plans cover regular services such as checkups and cavity treatment (with deductible or co-pay). However, more expensive treatments needed for services such as fabrication and placement of crowns and bridges may be subjected to an annual dollar limit. With this in mind, those looking to purchase dental insurance should be careful to look over the fine points of any potential plans they are considering. It is also very important to identify which dental services are not covered. All too often, patients assume that their plans cover services that their insurance providers do not include. It is far better to know these limitations in advance than to face unexpected charges. Our office staff can help.

P.S. It is important for patients requiring dental implants to know beforehand what their dental insurance covers, as well as what their regular health insurance might cover.

HEROES AND VILLAINS

DENTAL CC 7

One villain responsible for tooth decay is the bacterium streptococcus mutans, which breaks down sugar and produces the sticky molecules (glucans) that are responsible for allowing bacteria to cling to our teeth. This villainous bacterium also produces an enamel-eroding acid responsible for cavities. The heroes that counter these effects are called “polyphenols,” which are found in the fermented stems, seeds, and grape skins left over from wine production. These compounds, which are also found in cranberries, block streptococcus mutans’ ability to produce glucans. As helpful as polyphenols are, however, no one is suggesting that we drink wine or sugary cranberry drinks to prevent tooth decay. Instead, scientists hope to isolate these compounds and put them in toothpaste and mouthwashes.

ARE YOU A GOOD DENTAL IMPLANT CANDIDATE?

DENTAL CC 6

Not only do dental implants look and feel natural, they have the potential to last a lifetime. These alternatives to bridges, crowns, and dentures owe their extraordinary stability to the fact that they fuse to the jaw bone. Dental implants also contribute to the health of surrounding teeth because they do not rely on adjacent teeth for support. Thus, as long as patients with missing teeth have healthy gums and adequate bone to support the implant, they are potential candidates for dental implants. A thorough evaluation by the dentist helps make the determination. Those best suited to this permanent tooth-replacement treatment have good overall oral health and are committed to taking care of their (implanted) teeth.

P.S. Smokers and those with poorly controlled diabetes are not good candidates for dental implants.

DIABETICS AND GUM DISEASE

DENTAL CC 5

Diabetics should be conscientious about visiting the dentist as often as recommended. According to a recent review of data from seven studies, patients with both diabetes and gum disease who were treated for their periodontal conditions over a four-month period displayed better glucose control than diabetic patients whose gum disease was left untreated. Periodontal disease, a bacterial infection of the gums that induces redness and inflammation, can be effectively treated with antibiotics and plaque removal by the hygienist. Plaque is the bacteria-laden, sticky substance that collects on tooth surfaces. This buildup may cause chemical changes that weaken diabetics’ ability to metabolize glucose. Thus, diabetic patients should be scrupulous about brushing and flossing at home and scheduling regular professional tooth cleanings.

March 28, 2011 column dental CC-5

Periodontal disease is often silent, meaning symptoms may not appear until an advanced stage of the disease. Persistent swollen, red or bleeding gums, tooth sensitivity, and bad breath are among the warning signs of periodontal (gum) disease. If you need answers to a dental problem, or just want to improve your daily dental care, you'll find we have the information you need to keep your smile in top shape. P.S. Because plaque reforms on the gums on a continual daily basis, brushing and flossing daily is essential to staving off tooth decay and gum disease.

GUM TREATMENT MAY REDUCE PREMATURITY RISK

DENTAL CC 4

Pregnant women should make visits to the dentist part of their overall prenatal-care regimen. According to new research, doing so may reduce the risk of premature birth, which is associated with periodontal (gum) disease. This new study may alter the way that dentists treat gum disease in pregnant women. The bacterial infection associated with gum disease breaks down gum tissue and leaves it chronically inflamed. This is a particular problem during pregnancy because hormonal changes appear to make pregnant women more susceptible to periodontal disease. Yet, standard tetracycline-based therapy used to treat it is considered to pose a risk to the baby. However, aggressive non-drug tooth cleaning (root scaling and planing) is safe and reduces the risk of premature birth.

P.S. Root scaling and planing involves removal of tartar and plaque that attach to tooth surfaces and targets areas below the gum line (along the root) in particular.

THE DENTAL FACELIFT

DENTAL CC 3

The “dental facelift” is a non-surgical approach to improving the contours and dimensions of the face, with the comprehensive and coordinated application of veneers on tooth surfaces. As a result of this cosmetic procedure, patients not only emerge with more youthful, whiter smiles, they have added volume to their faces that helps fill in sunken cheeks and shrunken lips and jaws. To understand how this can be accomplished, it is important to look at the teeth as playing a major role in the framework of the face. As we age, teeth wear down and the jaw recedes, which leads to decreasing facial height and a ¬slackened appearance. By building up tooth surfaces with veneers, the face can be subtly reshaped.

March 14, 2011 column dental CC-3

Veneers are ideal for teeth that are too small, too big, or have uneven surfaces. Feel free to contact us with any questions about veneers, or to schedule a consultation. At JOLLY SMILES, we believe it is our responsibility to share our acquired knowledge and understanding of oral health with each and every patient, one on one. JUST TEETHING

DENTAL CC 2

“Teething” is the term used to describe the emergence of a baby’s first (primary) teeth through his or her gums. As might be imagined, this can be a painful experience for children between the ages of four and seven months. If your baby’s teeth emerge later, bear in mind that there is a great deal of variability to eruption times. Aside from drooling, crying, and other signs of irritability, a teething baby may have a low-grade temperature of up to 100 degrees Fahrenheit. Any temperature above this level should not be considered to be associated with teething and should be brought to the attention of the pediatrician. To allay the pain, try offering your child a frozen teething ring.

P.S. Parents, take heart: For some babies, teething is painless.

GETTING THE FULL TREATMENT

DENTAL CC 1

Fluoride treatments during regular professional cleanings make sense for adults because they still get cavities. This is especially true at the root surfaces and under crowns, which become exposed as adult gums recede. Particularly susceptible to cavities are root surfaces because they lose mineral faster than enamel does. With this in mind, higher concentrations of fluoride provided by topical treatments may be needed for adequate protection. It should also be noted that many adults take certain medications and have medical conditions that cause dry mouth, which is a condition that increases the risk of cavities in adults. Fluoride makes teeth more resistant to acids produced by bacteria and helps remineralize tooth surfaces that are under attack.

February 28, 2011 column dental CC-1

Fluoride is a mineral that occurs naturally in many foods and water. Could you benefit from additional fluoride? At JOLLY SMILES, we will make suggestions to help you maintain good oral health. WHICH WINE STAINS TEETH THE MOST?

DENTAL BB 60

Patients wanting to preserve the whiteness of their teeth may wonder what role wine plays in staining tooth enamel. According to a recent study, red wine causes immediate and readily apparent staining while white wine makes teeth more susceptible to staining later on. The red pigments in red wine cause stains on contact, but more acidic white wines actually etch the outer surface of teeth slightly, which sets them up to accept staining pigments. So, red wine drinkers will stain their teeth right away, and white wine aficionados will notice tooth stains later. With this in mind, drinkers should be careful not to follow a glass of white wine with coffee, tea, cigarettes, or other staining agents.

TAKING IT ON THE JAW

DENTAL BB 59

A serious potential consequence of periodontitis (advanced inflammation or infection of the gums) is tooth loss due to jaw-bone shrinkage and damage. The latest effort to counter this threat comes in the form of a drug called teriparatide, which has recently been proven to regrow bone in jaws. According to one study, patients with tissue damage caused by periodontitis developed nearly ten times more bone with teriparatide treatment than those receiving daily placebo injections. The significant gains in bone produced by the drug developed over a relatively short six-week period. These improvements were sustained, and even improved upon, over a succeeding 12-month period. This jaw- regrowing ability may also prove useful for potential tooth implant patients, who need more foundation bone.

P.S. Periodontitis is a major cause of tooth loss that affects more than one in five U.S. adults.

HEALTHY HEART REGIMEN WITH REAL TEETH

DENTAL BB 58

According to various studies, estrogen helps prevent atherosclerosis and helps women stave off heart disease until they reach menopause. At that point, as estrogen production wanes, women begin to catch up with men in terms of heart disease risk. The good news is that older women who receive regular dental care have a one-third less risk of developing heart disease than women who don’t pay close attention to their oral health. That is the conclusion reached by researchers who analyzed the medical records of nearly 7,000 adults aged 44 to 88. Although the study found no beneficial link between regular dental care and heart disease prevention in men, women were found to benefit by having fewer heart attacks and strokes.

P.S. There is a growing body of evidence to show that gum disease, which is the most common chronic inflammatory condition in the world, is linked with heart disease.

BABY-BOTTLE TOOTH DECAY

DENTAL BB 57

Few things relax infants more than sucking milk or fruit juice from a bottle. When babies are allowed to suck on a bottle at bedtime until they go to sleep, however, this habit can prove to be potentially harmful. Bathing teeth in a sugary liquid in this manner promotes tooth decay. According to various studies, as many as 11% of pre-school children have cavities caused by sucking on a bottle. If a bottle-fed child’s mouth is not properly cleansed after sucking on a bottle by lightly swabbing his or her teeth and gums with a water-dampened cloth, plaque will accumulate to cause cavities. If your child must have a bottle at bedtime, provide one filled with water.

January 31, 2011 column BB 57

Early dental visits can keep watch on how tooth development is progressing. They are an important part of growing up healthy. When it’s time for their first visit, satisfy your children’s natural curiosity by telling them that the dentist is first going to count their teeth, then take their picture, clean them, and see that they are healthy. Bring along a doll or stuffed animal, and we’ll check its teeth too. At JOLLY SMILES, we believe that a healthy smile is important at any age. Our office is located at 102 Sleepy Hollow Drive, Suite 100. Call us at 302-378-3384 for an appointment. Evening hours are available.

P.S. Mothers who breast-feed their children must also clean their babies’ mouths with a damp cloth.

REPAIRING CHIPPED TEETH

DENTAL BB 56

Dentists across the nation are reporting more instances of chipped teeth among their patients as a result of the tooth-grinding that is thought to be a symptom of tough economic times. Fortunately, there are easy fixes for this problem. To begin with, the dentist may reshape a chipped tooth by removing small amounts of enamel (enamel recontouring) to smooth and even out the tooth outline. Otherwise, bonding may be a more suitable option. This tooth-restoration technique involves painting a composite resin directly onto the tooth. Subsequent layers can be added to fill in the space left by the chip. Finally, the bonded portion of the tooth is sculpted to restore the tooth to its original shape.

P.S. A tooth that is badly chipped may require restoration with a porcelain veneer.

2 + 2 = HEALTHY TEETH AND GUMS

DENTAL BB 55

By adhering to the “two plus two” rule, patients can help better their oral health. Most dentists recommend that at least “two” minutes of tooth-brushing is required to thoroughly clean teeth. Although many people think they are following the two-minute tooth-brushing regimen, actual timing of their routines may prove otherwise. If necessary, patients should use an old-fashioned egg timer to time themselves. Flossing is also a must at least once a day because it reaches between teeth where toothbrush bristles cannot go. Regarding regular visits to the dentist, “two” visits a year is acceptable for most patients, while visits every three to four months is preferable for those with bridges, crowns, or a history of gum disease.

January 17, 2011 column BB 55

A very good lesson to remember is that prevention is paramount to optimum dental health. Through good habits like proper brushing and flossing, regular professional visits, and choosing a dental-friendly diet, you have a lot of control over your dental health. One of the most important services we offer at JOLLY SMILES is a plan for preventive dental care, including counseling on the proper way to brush and floss and recommendations for oral hygiene products and how to use them. For an appointment, please call us at 302-378-3384, 102 Sleepy Hollow Drive, Suite 100. Evening hours are available.

P.S. Brushing the tongue is important because that is where the bulk of bacteria collect that cause bad breath.

PREVALENCE OF GUM DISEASE HIGHER THAN ESTIMATED

DENTAL BB 54

The incidence of gum disease in U.S. adults may have been underestimated by as much as 50 percent. This surprising news comes from a pilot study funded by the Centers for Disease Control and Prevention’s Division of Oral Health, which conducted full-mouth, comprehensive, periodontal examinations on more than 450 adults over age 35. When the findings were compared with previous (partial-mouth) studies, it was found that the prevalence of moderate to severe gum disease may have been underestimated by 50 percent. This finding has serious implications because periodontal (gum) disease is an inflammatory condition that not only can lead to tooth loss, but it has been linked with other chronic inflammatory diseases including diabetes, cardiovascular disease, and rheumatoid arthritis.

P.S. Adults should have their teeth checked on a yearly basis as well as undergo comprehensive exams to assess their periodontal disease status.

YOU KNOW THE DRILL! OR DO YOU?

DENTAL BB 53

Many patients think that drilling away decay and filling cavities in teeth is an all-or-nothing matter. Those patients who hold this view may be surprised to learn that they have it in their power to reverse some instances of dental decay and avoid the drill entirely. The key point to remember is that regular dental checkups enable the dentist to identify tooth decay in its earliest stages. At this point, when the acid produced by bacteria-laden plaque has only partially penetrated the outer enamel surface, the damage can be reversed with better adherence to home oral care (brushing and flossing) and applications of fluoride that encourage calcium and phosphates in the saliva to “remineralize” the tooth surface.

January 3, 2011 column BB 53

Just as dental decay and gum disease are your teeth’s worst enemies, routine brushing, flossing, and professional checkups are your teeth’s best friends. Has it been so long since your last dental visit that you can’t remember when it was? Then why not take charge of your dental health and call us at JOLLY SMILES, 302-378-3384, for an appointment. At 102 Sleepy Hollow Drive, Suite 100, we are committed to delivering the most current state-of-the-art care in a relaxed, friendly environment. Most insurance plans accepted.

P.S. A drill-less procedure recently introduced to the United States from Germany, called Icon, treats “intermediate” cavities with a clear resin that prevents the cavity from progressing.

IT’S A MATCH!

DENTAL BB 52

When filling a cavity, the best results are achieved with a material that not only matches the original tooth color, but actually strengthens the structure of the tooth. All this is made possible with composite (resin) fillings that can be matched precisely to a tooth’s specific color. As a result, there is no unsightly tooth-darkening so often associated with traditional amalgam (mercury) fillings. This is a particularly important consideration when filling cavities in visible areas of the front teeth. Composite fillings are also sufficiently strong for use in back teeth. Because the resin integrates with the existing tooth, the material actually strengthens the tooth’s structure. Moreover, composite fillings allow for more precise tooth preparation and are resistant to temperature extremes.

December 20, 2010 column BB 52

Composite resin restorations and other advances in cosmetic dentistry make it harder than ever to tell the difference between what’s natural and what’s not. They also make it easier than ever to smile with confidence. This column is prepared as a public service to help our Middletown family and friends become aware of and better understand modern dentistry and the power of a healthy smile. INFLAMMATORY SITUATION

DENTAL BB 51

Not only does the buildup of the sticky, bacteria-laden material known as “plaque” increase the incidence of decayed tooth enamel, it also promotes “gingivitis” (inflammation of the gums). When these sticky deposits of bacteria, mucus, food particles, and other irritants accumulate around the base of the teeth, they exert a toxic effect that causes the gums to become irritated, red, and swollen, and to bleed more easily. These symptoms are all signs of gum disease. Left untreated, gingivitis can develop into the more serious condition called “periodontitis,” inflammation of the membranes around the base of the teeth, which can lead to erosion of the bone holding teeth in place and resultant tooth loss. To avert these potential problems, brush and floss daily.

P.S. Because plaque collects on teeth on a daily basis, it is necessary to brush and floss at least twice daily to rid teeth of this potentially harmful material.

GOING THROUGH A ROUGH PATCH

DENTAL BB 50

Sores in the mouth that linger for more than a week should be examined by the dentist. “Leukoplakia,” a thick, whitish patch that occurs on the cheeks, gums, or tongue, is caused by irritation due to rough teeth or rough surfaces on dentures, fillings, and crowns. It may also result from smoking or other tobacco use (smoker’s keratosis). Because it can progress to cancer, the dentist may want to take a biopsy. Patients should also contact the dentist for an immediate examination if they should notice a persistent sore or irritation that does not heal, color changes (such as the development of red and/or white oral lesions), or pain or tenderness anywhere in the mouth or lips.

P.S. The typical white patch of leukoplakia develops slowly over weeks to months.

BRIDGE BUILDING

DENTAL BB 49

A “bridge” is a fixed dental restoration that is commonly used to replace no more than four adjacent front teeth or two side teeth. Much like a highway bridge, it is supported by two abutments, which are structurally healthy teeth on either side of the gap. The span, or “pontic,” is a metal framework attached to the abutments at each end with the necessary number of attached artificial teeth. When properly crafted, the artificial teeth and abutment crowns should match the contour and color of natural teeth and appear to be growing out of the gums. However, the pontic should barely touch the gums because exerting unnecessary pressure on the gums and underlying bone can damage them.

December 6, 2010 column BB 49

If one or more of your teeth have been lost to damage or decay, prompt attention with bridges, partials, implants, or other methods of repair will prevent additional, perhaps bigger and more painful problems later on. Visit your dentist on a regular basis, and practice good home oral hygiene. It will pay off in the long run. If you don’t have a family dentist at this time, we invite you to JOLLY SMILES, 102 Sleepy Hollow Drive, Suite 100. We have convenient office hours by appointment. Call 302-378-3384. We accept most insurance plans.

P.S. There should be sufficient space on the underside of the pontic of a dental bridge so that dental floss can be passed beneath it to keep it clean.

FEWER TEETH RAISES HEART DISEASE RISK

DENTAL BB 48

As scientists are coming to understand the interplay between various organs and systems of the human body, we are learning that no part of the body really functions alone. The latest example of this interdependency involves the finding that cardiovascular disease (and coronary heart disease, in particular) is closely related to the number of teeth that a person has left. According to recent research, a person with fewer than ten of his or her own teeth has a seven times higher mortality risk by coronary heart disease than a person of the same age and of the same sex with more than 25 teeth left. The common denominator between fewer teeth and heart disease is thought to be chronic inflammation.

November 22, 2010 column BB 48

Keeping up with regular dental checkups will give your dentist the opportunity to watch for areas of inflammation and developing gum disease. And if you conscientiously follow your dentist’s recommendations for at-home care, you will be more likely to have fewer potential gum problems. When was your last dental visit? Now would be a good time to schedule your next exam. If you don’t have a family dentist, we are accepting new patients at JOLLY SMILES, and we welcome your call at 302-378-3384. We’re located at 102 Sleepy Hollow Drive, Suite 100. Evening appointments are available.

P.S. The best way to control inflammation in the mouth is to guard against gum disease.

BABY TEETH WARRANT EARLY EXAMINATION

DENTAL BB 46

Perhaps because they know that they are temporary, parents often do not regard their children’s “baby teeth” with the same amount of respect they give to the permanent teeth that replace them. As a result, parents usually wait until their children’s primary teeth begin to appear before they make an appointment for them to visit the dentist. In fact, a poll by the American Academy of Pediatric Dentistry reveals that nearly three- quarters of parents (70%) wait until their children are three years old before scheduling a dental appointment. Dentists recommend that parents schedule their child’s first dental exam by age one or within six months of the eruption of the first baby tooth.

P.S. During a child’s first dental exam, the dentist can check for signs of early childhood (“baby bottle”) decay, educate parent and child on proper oral hygiene, and establish a comfortable relationship with the young patient.

FLOSSING FOR APPEARANCE’S SAKE

DENTAL BB 47

After brushing but not flossing, about 40% of tooth surfaces will continue to harbor sticky bacteria (plaque) that give rise to tooth decay and gum disease. If this percentage does not motivate everyone to floss daily, it should be noted that failure to floss can also make a person look older. Overgrowth of plaque creates gum inflammation that can lead to gum recession and bone loss. In fact, the term for aging “long in the tooth” derives from receding, uneven gumlines that make people look older. On top of that, when gum disease leads to bone loss, lips and cheeks cave in and wrinkles form. This puckered, hollowed-out look cannot necessarily be fixed with cosmetic surgery. Prevention is best.

P.S. Once a tooth is lost, there is no reason for the underlying bone to continue to exist.

SAME-DAY TOOTH IMPLANTS

DENTAL BB 45

While traditional dental implants usually require up to six months for the titanium implant to successfully bond with the surrounding bone (“osseointegration”), same-day dental implants don’t require a long waiting period. The ability of these “immediate-load” tooth implants to be readily available as foundations for replacement teeth is a direct outcome of their advanced design. As a result, patients can leave the dental office with an immediate-load implant on the same day they have a tooth extracted. With preplanning, it is even possible to have a permanent replacement tooth in place instead of a temporary. Same-day tooth implants cut down on office visits, spare patients the embarrassment of enduring visible gaps, and ensure excellent gum interface with the replacement tooth.

November 8, 2010 column BB 45

Wouldn’t you like to bite into a sandwich knowing your dentures won’t slip? Go to sleep each night without removing your teeth? Smile with confidence? Same-day implants may be the answer for you. Implants are an exciting tooth-replacement option that has changed the lives of many people. In fact, more than a million have been placed in North America alone. Are they for you? We welcome your call at JOLLY SMILES for a consultation about your future smiles. You can reach us at 302-378- 3384, 102 Sleepy Hollow Drive, Suite 100. Most insurance plans are accepted.

P.S. As with dental implants, immediate-impact implants are not suitable for all patients. Consultation with the dentist can help patients make the tooth-replacement decision that best suits their needs and circumstances.

GONE IN A PUFF OF SMOKE!

DENTAL BB 44

If you think that smoking’s damage to health is limited to the lungs and heart, think again. Smoking also increases the risk of tooth loss. In fact, recent research on the subject indicates that smokers have twice the risk of losing their teeth than non-smokers. According to researchers, after accounting for variables such as age, gender, and coffee/alcohol consumption, smokers were found to lose significantly more teeth than their non-smoking counterparts. Researchers speculate that the reason for this is that smoking damages the bone that holds teeth in place. As a result, smokers not only develop wrinkles and creases around their mouths, they may also suffer a vertical collapse of their lower faces due to lost teeth.

P.S. Smoking increases the risk of developing oral cancer.

COSMETIC CONCERNS

DENTAL BB 43

It hardly comes as a surprise that cosmetic, or aesthetic, dentistry has grown to be so popular among patients. After all, straight, white teeth are as essential to appearance as nice skin and coiffed hair. The most sought-after procedure is tooth-whitening, which can be performed in- office, at home, or a combination. The most aggressive yet painless tooth-whitening techniques result in smiles several shades whiter in one office sitting. Next on the list of patient popularity are veneers used to correct cracked, chipped, discolored, and worn teeth. These ultra-thin tooth coverings made of porcelain or composite materials look like natural teeth, only they are perfect. If a less-expensive option is desired, resin bonding provides a simple, effective method of tooth supplementation.

P.S. Because porcelain and ceramic tooth replacements look like real teeth, they blend beautifully with existing teeth.

NUTRITION AND OBESITY

DENTAL BB 42

The increasing incidence of obesity in this country has many people concerned, and none more so than parents of overweight children. As it turns out, the dentist may help parents educate themselves and their children about the role that nutrition plays in attaining normal weight. Researchers have found that among young children between ages two and five years, poor nutrition may be the common denominator that links obesity and tooth decay. In fact, 28 percent of young children in this age group, who have decay serious enough to require anesthesia for treatment in an operating room, had a “body mass index” (BMI) that indicated they were overweight or obese. Better nutrition may help avert both obesity and severe tooth decay.

P.S. Processed snacking foods not only tend to be highly caloric, but they are often made of sticky sugars and carbohydrates that adhere to teeth and contribute to tooth decay.

THE GUM DISEASE/PRETERM BIRTH LINK

DENTAL BB 41

While research has already established a link between periodontal (gum) disease in pregnant women and an increased risk of giving birth to babies of low birth weight prematurely, a new study provides additional news on the link between gum disease and premature birth. According to researchers, pregnant women who undergo successful treatment are less likely to give birth early. This finding is based on a study of nearly 900 pregnant women, with and without gum disease, who were pregnant for between six and twenty weeks. This study provides a good argument for women to incorporate dentists into their prenatal healthcare team. Maintaining good oral health before and during pregnancy is in the best interests of both mother and child.

P.S. So far, research suggests that periodontal disease triggers increased levels of biological fluids that induce labor.

GET YOUR TEETH INTO BETTER SHAPE!

DENTAL BB 40

Have you ever thought of your teeth as being either misshapen or too big? If you have, “tooth reshaping” is a cosmetic procedure that involves the removal of small amounts of enamel in order to minimize shape imperfections. For instance, the points of upper canine teeth can be blunted somewhat to eliminate their fang-like appearance. Tooth reshaping (or “enamel recontouring”) can alter the shape, length, contour, or position of a tooth and its relationship to adjacent teeth. The technique can also be used to reshape chipped, overlapped, or fractured teeth, or to minimize the appearance of crowding. All this can be accomplished without removing substantial amounts of tooth structure. Only one office visit and no anesthesia are required.

P.S. Tooth reshaping may be used to correct such developmental imperfections and abnormalities as pitting and grooves in tooth enamel.

EAT BETTER, AGE BETTER

DENTAL BB 39

While seniors generally consume fewer calories because of their slowing metabolisms, they must be sure to get sufficient calories to sustain their health. Weight loss at an advanced age for whatever reason may lead to malnutrition and/or worsening of existing health conditions. With this in mind, seniors should make sure their teeth (or dentures) and gums are in good working order. If it hurts to chew, people eat less. Healthy teeth and properly fitting dentures ensure that seniors can avail themselves of all the nutrition in healthy meals. The National Institutes of Health is funding research to determine how oral health may affect memory. It is thought that a healthy diet may be linked with less cognitive decline among seniors.

MATTERS OF THE HEART

DENTAL BB 38

Not only are brushing and flossing essential for good oral health, they may also be good for the heart. So say the researchers who found that failure to brush one’s teeth twice a day increases one’s risk of heart disease. Specifically, the study of nearly 12,000 adults showed that those with poor oral hygiene had a 70 percent higher risk of heart disease than those who brushed twice daily and were less likely to have unhealthy gums. While failure to brush one’s teeth may not be in the same league as smoking when compared to a heart disease risk factor, the simple habit of tooth-brushing can make a difference in combating the number one cause of death in this country.

IS TOOTH REGENERATION IN THE FUTURE?

DENTAL BB 37

Each year, dentists treat millions of cavities that result from the enamel-eating acids emitted by sugar-eating bacteria missed by flossing and brushing. While drilling out tooth decay and filling cavities with amalgam or composite fillings has proven extremely effective in preserving the remaining healthy tooth structure, patients often wish they could somehow magically eliminate any decay and simply regenerate the tooth back to its previous healthy state. That wish may someday be a reality because scientists have identified a peptide known as MSH (melanocyte-stimulating hormone) that encourages cells within decayed teeth to regenerate in about one month. When introduced into the teeth in a gel or film in the laboratory, cavities disappeared and were replaced by healthy tooth structure.

P.S. MSH (melanocyte-stimulating hormone) has not demonstrated any ability to prevent cavities, which means that it is no substitute for flossing and brushing.

INLAYS AND ONLAYS

DENTAL BB 36

Among the cosmetic dentistry procedures that provide restorative benefits are inlays and onlays made from porcelain or composite materials. Dental inlays are used to treat teeth with decay within their indented top surfaces. Onlays are used to treat decay or damage that extends to one or more tooth cusps. Both restorations usually require two appointments. One involves taking an impression and placement of a temporary inlay or onlay in or over the tooth. Once the inlay or onlay comes back from the lab, where the impression was used to create the restoration, it is cemented in place. Not only do inlays and onlays require less tooth reduction than a crown, they are durable enough to last up to three decades.

August 30, 2010 column BB 36

We will be glad to discuss inlays and onlays and other options available to keep your smile a beautiful, happy one. At JOLLY SMILES, we believe in an informed patient, and we promise to provide you with information regarding all aspects of your dental care. GETTING TO THE ROOT

DENTAL BB 35

In the event of a tooth becoming abscessed (due to infection) or traumatized (due to injury), it is often necessary to perform root canal therapy to remove the dead or damaged root pulp. The goal of the procedure is to save the crown (white outer portion of the tooth). The pulp is the area inside the crown that extends down into the gums with nerves and blood vessels. The dentist can check to see if the tooth is alive by testing whether the patient can feel cold on the tooth or reacts to an electrical stimulation on the tooth. The procedure involves removing the pulp from the non-vital tooth and filling and sealing the area and securing the crown.

August 16, 2010 column BB 35

Although some say that anticipation is half the fun, expecting root canals to be painful is usually more than half the reason for a patient’s fears. We hope you’ll feel free to discuss your dental concerns with us at JOLLY SMILES. You can always count on us to be truthful and compassionate about your dental care. If you’ve never visited our office before, you may not realize how pleasant a visit to the dentist can be, since we pay as much attention to the simplest six-month cleaning as we do to more complex issues like root canals. SHARED CONCERNS

DENTAL BB 34

Parents may want to take note of the fact that they can pass along cavity-causing bacteria to their babies. This information is provided in the interest of raising awareness, not causing alarm. What many people do not know is that Streptococcus mutans is a bacterium that can be transferred by saliva when utensils are shared and even when parents kiss their babies. It’s important to note that only parents and caregivers with active tooth decay can spread Streptococcus mutans bacteria when saliva is transferred. Beyond that, tooth decay develops as a result of a combination of factors, including genetics, oral hygiene, and exposure to sugary liquids. Good oral health on the part of parents helps minimize the bacteria-transfer factor.

P.S. Babies are particularly vulnerable to tooth decay during the time of tooth eruption, when their tooth enamel is relatively soft.

STRINGING ALONG

DENTAL BB 33

The reason that dentists insist that their patients floss (as well as brush) their teeth is because flossing does nearly half the work necessary to remove “plaque.” Unless this sticky bacteria-laden substance is removed from between teeth, where toothbrush bristles may not reach, tooth decay and gum disease will result. Patients with receding gums or large spaces between their teeth are advised to use a flat, wide dental tape. If teeth are closely spaced, thin floss is best, especially non-shredding floss. Patients with bridges and braces are likely to find it necessary to use a floss threader to get underneath restorations or wires between teeth. There is also floss with a stiff end that does the job equally well.

P.S. Patients who do not have the dexterity needed to use dental floss may want to try soft wooden plaque removers.

WHERE THERE’S SMOKELESS, THERE’S FIRE

DENTAL BB 32

Regular users of smokeless tobacco should understand that they face much the same risk of developing cancer as their tobacco-smoking counterparts. In 1986, the Surgeon General warned that the use of smokeless tobacco “is not a safe substitute for smoking cigarettes. It can cause cancer and a number of noncancerous conditions and can lead to nicotine addiction and dependence.” Both chewing tobacco and snuff (shredded tobacco in a pouch) contain 28 carcinogens (cancer-causing agents), the most harmful of which are the tobacco- specific nitrosamines (TSNAs). The National Cancer Institute warned that nitrosamines, which are present in smokeless tobacco at relatively high levels, are not safe at any level. Smokeless tobacco users increase their risk for cancer of the oral cavity.

Recent research has uncovered links between gum disease and other parts of the body that previously may have seemed unlikely. For instance, periodontitis has been associated with an increased risk of heart disease and stroke, which makes a good case for addressing gum inflammation. More recently, researchers have discovered another potential benefit of keeping gum disease at bay. According to research, it has been found that treating periodontal disease in diabetics may lower their insulin levels. Researchers suggest that the connection is based on bacterial infections of the mouth that cause inflammation, which results in chemical changes that reduce the effectiveness of insulin produced in the body. As a consequence, diabetics find it more difficult to control their blood sugar.

August 2, 2010 column BB 31

PROMOTING HEALING AFTER TOOTH EXTRACTION

DENTAL BB 30

Once a tooth is extracted, the dentist likes to see good healing that leads to bone formation. Otherwise, poor healing may lead to excessive bone loss that compromises the patient’s ability to accept a dental prosthetic or implant. Consequently, there is reason to cheer a recent study that shows that platelet rich plasma (PRP) therapy accelerates post- extraction healing and bone formation. PRP therapy magnifies the body’s first response to soft-tissue injury, which is to deliver platelet cells. These blood components are packed with growth and healing factors that initiate repair and stimulate stem cells. By injecting large concentrations of the patient’s own platelets into the extraction site, it is possible to jump start the body’s natural healing response.

To keep your teeth and to keep your teeth strong, be sure to keep your dental appointments for regular checkups. Remember that nothing is more economical than regular, professional visits to the dentist to keep any problems at bay. If you have questions about caring for your teeth and gums and achieving optimum dental health, please feel free to call us at JOLLY SMILES, 302-378- 3384. Our full-service, state-of-the-art office is located at 102 Sleepy Hollow Drive, Suite 100, where we have office hours by appointment. Most insurance plans are accepted.

P.S. In the study mentioned above, PRP therapy increased bone density as much in the first week following tooth extraction that untreated control sites took six weeks to reach.

COSMETIC BENEFIT OF DENTAL IMPLANTS

DENTAL BB 29

Aside from restoring biting and chewing capability to the mouth, dental implants also serve a cosmetic function. The most obvious benefit to an implant patient’s appearance is that the replacement tooth fills in the gap left by the lost tooth with a natural-looking substitute. On a deeper level, dental implants help patients avert bone loss. Without implants, missing teeth and associated bone loss cause the lower third of the face to collapse and shrink inward. As the anchoring portion of a dental implant forms a strong bond with the jaw, it stimulates the surrounding bone and preserves it. As a result, the bone loss that would otherwise be inevitable with missing teeth is avoided.

July 19, 2010 column BB 29

In addition to their cosmetic appeal, implants eliminate the day-to-day frustrations and discomfort of ill-fitting dentures. They allow people to enjoy a healthy and varied diet without the restrictions some denture wearers face, and implants often bring a renewed sense of self-confidence. People with implants often say they feel better, look better, and live better. If you think implants may be for you, call us at JOLLY SMILES for a consultation. A visit to 102 Sleepy Hollow Drive, Suite 100, may be just what you need to enhance your quality of life. Call us at 302-378-3384.

LASER DENTISTRY

DENTAL BB 28

The future of dentistry has actually been available for some time, in the form of an advanced laser instrument that can be used in place of a drill to perform a variety of dental procedures. Waterlase® Dentistry features a handheld instrument that combines a spray of atomized water with laser energy to conduct procedures ranging from tooth-decay removal and enamel etching to gum reduction, without the heat, pressure, vibration, or pain often associated with a dental drill. As a result of the remarkable degree of precision and comfort afforded by this advanced laser technology, patients often can forego the need for local anesthetic. Waterlase® Dentistry represents a large step forward in cavity preparation, root canal treatment, smile design, and other procedures.

July 5, 2010 column BB 28

Modern dental technology is the result of years of research by professionals dedicated to making dental care effective, efficient, long-lasting, and as painless as possible. ON PINS AND NEEDLES

DENTAL BB 27

Thanks to a host of technical and procedural developments, most patients view a visit to the dentist with little, if any, trepidation. However, if you are wary of going to the dentist, you are not alone. It is estimated that one in 20 people suffers from severe anxiety about dentistry (known as odontophobia), and about one-third of all people say they experience moderate anxiety when visiting the dentist. Fortunately, the dentist can effectively work with anxious and fearful patients who make their concerns known. Patients can avail themselves of treatments such as relaxation techniques and tranquilizers/sedation to lower their tension levels. Recent research shows that the ancient Chinese treatment of acupuncture also effectively quells patient anxiety and pain.

P.S. According to a recent study, dental patients who underwent five minutes of acupuncture treatment saw their Beck Anxiety Inventory scores drop from 26.5 to 11.5.

DISPLAYING SENSITIVITY

DENTAL BB 26

If you have sensitive teeth, you are hardly alone. It is estimated that 40 million Americans share your discomfort. Fortunately, the cause of most cases of tooth sensitivity has been identified, and the cure may be as easy as modifying your tooth- brushing technique. According to a nationwide survey of 700 dentists, aggressive brushing and acidic foods and beverages are the leading culprits when it comes to tooth sensitivity, which results from nerve irritation. Other contributing factors include the use of certain toothpastes, mouthwashes, and tooth-whitening products, as well as cracked teeth, bulimia, and acid reflux disease. A switch to a desensitizing toothpaste and a toothbrush with softer bristles usually helps relieve the problem.

P.S. If you drink acidic soft drinks and fruit juices, avoid brushing your teeth immediately after drinking, when enamel is most vulnerable.

FULL MOUTH RECONSTRUCTION

DENTAL BB 25

In some cases, patients’ teeth may be so decayed, injured/fractured, or worn/eroded that they may require extensive treatment known as “full mouth reconstruction.” Also known as “full mouth restoration” and “full mouth rehabilitation,” this rebuilding/restoring process typically involves every tooth in both the upper and lower jaws. The process begins with a thorough examination and evaluation of the patient’s teeth, gums, and occlusion (bite). It continues with an assessment of the color, shape, size, and proportion of the teeth with respect to how they relate to the gums, lips, mouth, side profile, and face. Then, after taking x-rays, impressions, and photographs, the dentist (in conjunction with specialists, if necessary) can recommend procedures and techniques for totally restoring the teeth.

June 21, 2010 column BB 25

Whether it involves full mouth reconstruction, simple fillings, implants, dentures, or cosmetic work, any program we suggest for your good dental health will be thoroughly discussed with you before we begin. When you are looking for state-of-the- art dentistry from people who really care, welcome to JOLLY SMILES. Our dental team works smoothly and efficiently together, devoted to making your dental experience comfortable as well as enhancing. Call us at 302-378-3384 for an appointment at 102 Sleepy Hollow Drive, Suite 100. Evening hours are available.

P.S. Full mouth restoration can include procedures such as crown lengthening; gum recontouring; jaw surgery; permanent restorations (crowns, bridges, inlays/onlays, veneers, and implants); and orthodontics.

GIVING BACK TO TEETH

DENTAL BB 24

Nearly everyone is aware that cavities form in teeth as a result of the acid produced by bacteria eating away at tooth enamel. What most people don’t know is that tooth enamel can also repair itself to some degree. Known as “remineralization,” the process occurs when calcium in the saliva (and from other sources) replaces lost calcium. In order for tooth enamel to repair itself in this manner, tooth surfaces must be scrupulously clean. There must also be a good supply of saliva and an adequate amount of available calcium. Patients can help themselves in two of these respects by brushing and flossing regularly and by consuming calcium-rich foods such as milk, yogurt, salmon, tofu, spinach, broccoli, peas, and Brussels sprouts.

P.S. Tooth enamel is composed of minerals, the most common of which is calcium phosphate.

PROFESSIONALLY SUPERVISE TOOTH WHITENING

DENTAL BB 23

Even if you’re considering using an over-the-counter tooth whitener, the American Dental Association recommends that you consult with your dentist. This recommendation is made because tooth-whitening systems containing carbamide peroxide or hydrogen peroxide can be uncomfortable for those with sensitive teeth or gum recession. In addition, most products will only whiten natural tooth enamel, which means that the presence of tooth-colored filings, veneers, crowns, or partial dentures may yield uneven results. With this in mind, the dentist can evaluate whether tooth-whitening is a good idea for your teeth. While tooth whitening usually reduces most stains caused by age, smoking, and regular coffee/tea drinking, it will not remove tooth discoloration caused by the use of tetracycline in children under age 8.

P.S. Tooth- whitening products are not recommended for children under age 16; pregnant women (or women who are breast-feeding); or people with sensitive teeth, gum disease, or worn tooth enamel.

LIVING IN STRESSFUL TIMES

DENTAL BB 22

There is anecdotal evidence from dentists across the nation that they are seeing a notably higher incidence of broken and cracked teeth, which most attribute to nighttime tooth grinding (“bruxism”). It can be argued that tough economic times have led many people to experience higher stress levels. The latest research on the subject shows that people who grind their teeth at night appear to be more likely to be suffering from stress. This is a problem because bruxism can lead to excessive tooth wear, looseness, and sensitivity. When dentists find evidence of these symptoms, they may recommend that patients wear custom-made mouth guards when they sleep. Such relaxation techniques as exercise and yoga are also recommended to reduce stress levels.

June 7, 2010 column BB 22

Bruxism isn’t only a nighttime activity. Some tooth-grinders continue this damaging practice throughout the day. Whether you are in our office for bruxism or just for a simple cleaning, our personal, casual, and relaxed style in providing quality dental care will help put you at ease. We’ll take the time to get to know your needs, answer your questions about the latest techniques and materials, and make sure that you are comfortable. Evening appointments are available. We accept most insurance plans.

P.S. According to the study mentioned above, stress-induced nighttime tooth grinding occurs in people across all age, gender, and education levels.

DENTURE- ADHESIVE ADVISORY

DENTAL BB 21

Denture- wearing patients should know that GlaxoSmithKline has ceased manufacture of their denture adhesives Super Poligrip Original, Ultra Fresh, and Extra Care over concerns of “potential health problems associated with the long-term excessive use of our zinc-containing denture adhesive products.” While the manufacturer states that their products are safe when used as directed, use of excessive amounts of the product over a period of years may pose a threat of neurological symptoms and blood problems (such as anemia) linked with ingestion of the adhesive. It should be noted that zinc is not absorbed through the mouth, but only when swallowed. As a precaution against unwanted effects arising from misuse of the product, GlaxoSmithKline plans to reformulate their products without zinc.

LIKE PARENT, LIKE CHILD

DENTAL BB 20

Youngsters of parents who make regular visits to the dentist are more likely to see the dentist themselves. According to research involving over 6,000 children between the ages of two and seventeen years, researchers found that about 86% of children whose parents had a dental visit during the preceding year had dental exams compared with 63% of children whose parents had not scheduled exams for themselves. With tooth decay as one of the most chronic diseases in the United States, parents are urged to focus on their families’ oral health needs. Treatment delayed is likely to be a good deal costlier, in terms of dollars as well as outcome, than immediate or preventive treatment of cavities and gum disease.

P.S. The American Dental Association recommends that children have their first visit to the dentist by their first birthday.

LASER TOOTH-DECAY DETECTION

DENTAL BB 19

While x-rays and probing instruments are important tools for identifying tooth decay, a laser that detects cavities can uncover even the smallest amounts of decay in the hardest-to-reach places. The DIAGNodent utilizes a laser-probe hand piece that emits pulsed light of a known wavelength on tooth surfaces. Once a baseline measurement of the patient’s healthy tooth surfaces is established, further examination of potentially problematic areas creates changing levels of fluorescent light, which the unit’s internal processor interprets. An audible signal warns of an indication of caries (tooth decay). The laser unit can also be used to keep track of early symptoms of tooth decay, which translates into earlier treatment and better tooth preservation.

May 10, 2010 column BB 19

Lasers have made a big impact on many medical specialties and have been used in dental procedures since the early 1980s. At JOLLY SMILES, ours is a unique, advanced facility designed to provide patients with the most up-to-date, comfortable, convenient dental care possible. PLASMA JETS INSTEAD OF DRILLS?

DENTAL BB 18

The future of dentistry may be more akin to science fiction than patients might ever have believed. In fact, it seems that new research involving streams of bacteria-killing plasma may replace today’s drills in the near future. Plasma is formed when electrons are emitted by energized gases. In this case, researchers used a relatively cool beam of purple plasma to eliminate oral bacteria in dental cavities. The plasma beam removed dentin (the material beneath the enamel that comprises the majority of the tooth) more selectively than a conventional drill. While this research is preliminary, researchers believe that plasma jets can be ready for removing tooth decay in as few as three to five years.

P.S. It isn’t the heat of the plasma that is thought to kill cavity-causing bacteria; it is the highly reactive molecules produced by charged oxygen molecules surrounding the plasma jet that get the job done.

OUT OF THE ROTATION

DENTAL BB 17

There are instances in which a permanent tooth will be rotated on its axis so that it isn’t aligned properly with either adjacent or opposing teeth. As a result of its twisted orientation, the rotated tooth creates a space on both of its sides where it should otherwise stand shoulder-to-shoulder with the teeth next to it. In cases where the rotation is not too extreme, the dentist can augment the rotated tooth with color-matching bonding material. This composite restoration is undertaken in an incremental manner to slowly fill in gaps and build up the tooth in areas where it is needed. All the while, care must be taken to ensure correct tooth dimension and attain the desired aesthetic result.

PREGNANCY AND GUM DISEASE

DENTAL BB 16

Dentists have long cautioned women about “pregnancy gingivitis,” which is caused by hormonal changes and affects more than half of all pregnant women. Usually, the gum inflammation associated with pregnancy-related gum disease occurs between the second and eighth months of pregnancy, and manifests itself in gums that look red and bleed somewhat during brushing. Without conscientious attention to good oral-hygiene habits and professional dental care, this mild form of gum disease (gingivitis) can lead to a more serious form of gum disease (periodontitis). If left untreated, gum disease poses risks to babies’ health such as premature birth, low birth weight, and even full-term stillborn birth. Pregnant women have more reason than their own oral health to seek regular dental care.

P.S. During pregnancy, increased levels of progesterone may encourage the growth of certain gingivitis-causing bacteria, render gum tissue more sensitive to plaque, and exaggerate the body's response to the toxins that result from plaque.

CORROSIVE EFFECT OF EATING DISORDER

DENTAL BB 15

The eating disorder known as bulimia poses a problem for the teeth that may be overlooked. Repeated purging of stomach acids can cause significant loss of tooth enamel. In one such case, a person with a history of bulimia began treatment to correct a closed bite, characterized by decreased vertical tooth dimension and an abnormal overbite in which the lower jaw protrudes. Due to stomach acid-induced enamel loss, the patient’s incisors lost one-third to one-half of their original length. The closed bite was caused by over-eruption of the lower incisors into the space created by eroded teeth. Fortunately, treatment of the eating problem led to aesthetic correction involving porcelain veneer crowns that could be used to restore the damaged teeth.

April 12, 2010 column BB 15

Porcelain veneers are just one option available today to change the look of your teeth. And teeth you are proud of can give you a new reason to smile. Don’t hide your smile behind a lack of information. A visit to JOLLY SMILES is an opportunity to find out new procedures as well as maximize your dental options. ORAL CANCER CHECK

DENTAL BB 14

In light of the fact that more than 34,000 Americans are diagnosed with oral cancer annually, it is a good thing that dentists regularly perform exams to detect cancers of the mouth. Such exams are very important since early detection leads to treatment, which leads to better outcomes. When oral cancer is detected in its earliest stages, the five-year survival rate is 80 percent to 90 percent. However, if oral cancer is left undetected for as little as a few months, the survival rate after five years drops to 55 percent. As with any disease, the best approach is prevention. All forms of tobacco products damage the DNA of cells in the mouth. Other risk factors include heavy alcohol consumption.

P.S. Alcohol consumption also compounds the risk of developing oral cancer posed by tobacco because a compound in alcohol (aldehyde) increases the absorption of cancer-causing substances in tobacco.

WISE CHOICE?

DENTAL BB 13

“Wisdom teeth,” which are the last teeth to erupt in the mouth, are somewhat controversial. While some people retain these teeth their whole lives, others choose to have them extracted, sometimes before they emerge from beneath the surface of the gums. The decision to remove wisdom teeth or leave them alone is one that should be made with the dentist. During an evaluation between ages 16 and 19 years of age, the dentist may recommend extraction if a wisdom tooth is crowding other teeth, causes pain, or has become impacted (stuck below the surface). Infection is another good reason to have a wisdom tooth extracted. Impacted wisdom teeth are often extracted by age 20 to prevent potential future problems.

March 29, 2010 column BB 13

Whether dealing with wisdom teeth, restorations, cosmetic dentistry, or dental implants, a good experience with dentistry is based on two things— choosing the right dentist whom you trust and who is up to date on the latest in dentistry, and taking the necessary steps to keep dental problems at bay through self-care at home between professional visits. At JOLLY SMILES, we work with you to maximize your oral health. Our office is located at 102 Sleepy Hollow Drive, Suite 100; Please call 302-378-3384 to schedule your appointment. Evening hours are available for your convenience. Most insurance plans are accepted.

P.S. If wisdom tooth extraction is recommended, it is best to do so before age 20, when developing tooth roots and bone are softer.

POCKET PRIMER

DENTAL BB 12

Gum disease results from accumulations of bacteria-laden plaque and tartar that cause infections in pockets in the collar of gum tissue that surrounds the teeth. Most of these bacteria are anaerobic, meaning that they can thrive without oxygen beneath the gumline. In the early stages of gum disease (gingivitis), the infections may be adequately treated with professional cleaning and conscientious home care. However, if the disease is allowed to progress to a more advanced stage (periodontitis), more aggressive remedial measures may be called for. Scaling and root planing is the procedure by which surfaces of the tooth roots are planed with scraping instruments until tartar is removed and tooth surfaces are sufficiently smooth for gum tissue to adhere to them.

March 15, 2010 column BB 12

It is estimated that more than half of all people over age 18 have at least the early stage of gum disease. After age 35, about three out of four adults are affected. Avoid the need for gum scaling and root planing, and protect yourself and your family—practice conscientious home care, have regular checkups, and eat a healthy diet. We’ll help too at JOLLY SMILES, because we care almost as much as you do about the health of your teeth and gums. Call us at 302-378-3384 for an appointment at 102 Sleepy Hollow Drive, Suite 100. Evening and Saturday hours are available. We accept most insurance plans.

P.S. If left untreated, periodontitis can lead to loosening, and loss, of teeth.

ROOT CAUSE

DENTAL BB 11

The reason that so many patients regard root canal (endodontic) treatment as painful is that they confuse the condition that makes treatment necessary with the treatment itself. If dental decay is not dealt with promptly, bacteria may infect tooth pulp. This inflammation or infection can, in turn, lead to an abscess if left untreated. Symptoms include aching or throbbing pain in the tooth. At this point, endodontic treatment is indicated. This procedure, which relieves painful symptoms, involves removal of the nerve and vascular tissue (pulp) from the root and pulp chamber, as well as any associated decayed tooth structure. After the inside of the tooth is carefully shaped and cleaned, it is filled, sealed, and ready for a crown.

March 1, 2010 column column BB 11

Root canal therapy may have had a bad reputation in the past as being painful, however, as most anyone who has experienced this procedure can attest, today’s techniques make root canal a virtually painless, effective treatment. If you are experiencing pain or have unusual symptoms, call your dentist. If you don’t have a family dentist at this time, we invite your call at JOLLY SMILES, 302-378-3384. You’ll find that our state-of-the- art dental practice at 102 Sleepy Hollow Drive, Suite 100, matches or exceeds your expectations of a pleasant, rewarding dental experience.

P.S. Root canal treatment spares a tooth with an abscessed root from extraction.

SUFFICIENT BONE FOR AN IMPLANT?

DENTAL BB 9

One of the primary requirements for successful dental implant surgery is a sufficient amount of bone to accommodate the titanium implant. In most cases, bone will usually fill in the socket left by an extraction within three months. However, if the walls of the socket are particularly thin, there may not be enough bone to accept the implant. This is particularly true of the upper and lower front teeth, where there may be only a thin ridge of bone (unlike the wide area of jawbone at the back of the mouth, where the molars reside). If so, a bone graft can be placed in the socket to help the body fill the hole left by the tooth root with bone.

P.S. A bone graft may also be recommended for patients who lost teeth many years ago and do not have enough bone left for an implant.

DRYING EFFECTS

DENTAL BB 10

Patients with symptoms of “dry mouth” should notify the dentist. Not only can reduced saliva production lead to difficulty speaking, swallowing, persistent bad breath, and chapped/cracked lips, it can also potentially contribute to cavities, gingivitis, and other dental problems. Once the dentist is made aware of dry mouth, there are ways to measure saliva production and evaluate the problem. Because dry mouth is a side effect of various drugs, a proper evaluation also entails a patient’s detailed description of any medications that he or she is taking. If dry mouth only occurs at night, it may be a symptom of a nasal obstruction that may require further medical evaluation. Dry mouth is also a symptom of some auto-immune disorders.

P.S. It is normal for saliva production to decline by as much as 40 percent as we grow older.

MAKING THE CEREC CHOICE

DENTAL BB 8

CEREC (Chairside Economical Restoration of Esthetic Ceramics) is CAD/CAM technology that utilizes a camera, computer, and milling machine to fabricate a precise tooth-colored, single- tooth restoration. This advanced computerized technique enables the dentist to create inlays, onlays, and full crowns in a single office visit without impressions, temporaries, or second office visits. This is accomplished by using a special camera to take a picture of the damaged tooth, the image of which is then displayed on a screen. The dentist next uses CAD (Computer Aided Design) technology to design a restoration that is then milled from porcelain using CAM (Computer Aided Manufacturing) technology. Within one hour, the patient can enjoy a beautifully restored tooth.

P.S. Because CEREC porcelain crowns contain no metal, there is no dark line at the gumline.

GOOD ORAL HEALTH PROMOTES CLEAR THINKING

DENTAL BB 7

There is already research that links gum disease with heart disease, stroke, diabetes, and Alzheimer’s disease. Now, a recent study suggests that gum disease may also be associated with poorer thinking skills. Researchers found that adults aged 60 and older with the highest levels of the gum disease-causing pathogen Porphyromonas gingivalis were three times more likely to experience trouble recalling a three-word sequence over time than those with lower levels of the pathogen. Adults with the highest levels of the pathogen were also two times more likely to fail three-digit reverse subtraction tests. It is thought that gum disease influences brain function through several mechanisms, including causing body-wide inflammation, a risk factor for loss of mental function.

P.S. As scientists slowly collect evidence that links oral health with brain function and overall health, patients have more reason than ever to brush and floss and visit the dentist regularly.

WHAT YOUR SMILE SAYS ABOUT YOU

DENTAL BB 6

Few aspects of your appearance say more about the “inner you” than your smile. Think of the difference in the reception you get when you smile at people as compared with those times when you withhold your smile. In fact, people often ask “What’s wrong?” when a person does not smile. Although it may not be intentional, that is the reaction that people elicit when they withhold a smile because they are embarrassed about the condition of their teeth. If you find yourself raising your hand to your mouth in a self- conscious effort to cover up your smile, you are needlessly depriving yourself of the attention you deserve from others. If so, cosmetic dentistry may help.

ADOPTING GOOD HABITS

DENTAL BB 5

Even before infants’ baby teeth erupt, parents can take steps to ensure their children’s oral health. Beginning with using a damp gauze pad to gently wipe baby’s gums, parents can remove residual milk, formula, and fruit juices from the mouth. These liquids contain sugars that bacteria feed on and produce acids that attack teeth. For this reason, a baby should not be allowed to fall asleep with a bottle of juice or formula in his or her mouth. Parents should start brushing their children’s teeth with a soft-bristled toothbrush and a pea-sized amount of toothpaste as soon as the first tooth appears. Flossing to prevent cavities between teeth should start when two teeth have erupted next to each other.

February 1, 2010 column BB 5

If it’s time for your child’s first visit to the dentist (around age 2 years), or if you can’t remember the last time you visited the dentist, it’s time to call your dental office. If you are looking for a family dentist, JOLLY SMILES is currently accepting new patients at our comfortable, state-of-the-art practice. And since February is Children’s Dental Health Month, created to increase dental awareness and stress the importance of regular dental care for children of all ages, we invite you to celebrate with us at 102 Sleepy Hollow Drive, Suite 100, and schedule an appointment for your child for a check-up. PH: 302-378-3384.

P.S. Poor oral health has been associated with the proliferation of unfriendly bacteria that, over a lifetime, can lead to heart disease and stroke.

CLEANING DENTURES

DENTAL BB 4

Dentures may be “false teeth,” but they still must be cleaned the same way that natural teeth are. As with real teeth, dentures are subject to buildups of sticky plaque, which poses the threat of infection and inflammation for gums. With this in mind, denture wearers are strongly urged to either brush their prosthetic teeth or soak them in cleaning solution. Arguably, the best cleaning method is brushing with toothpaste. However, for those who experience difficulty brushing their dentures effectively, soaking can remove as much plaque as brushing. For a short soak of about 15 minutes, enzymatic and effervescent solutions are probably equally effective at removing plaque. For longer soaks (up to eight hours), enzyme denture-cleaning solutions may be preferable.

P.S. Ultrasonic cleaning is about as effective as brushing when it comes to cleaning dentures.

STICKING TO YOUR GUMS

DENTAL BB 3

It is estimated that four out of five U.S. adults have some form of gum disease, which ranges from simple gum inflammation (gingivitis) to serious disease (periodontitis) that has the potential to damage bone and loosen teeth. For these reasons alone, adults have good reason to stop gum disease before it has a chance to cause severe damage. Without conscientious attention to brushing and flossing at home and regular professional cleanings, gums may become red and swollen, and bleed easily. In addition, the gums may form pockets around teeth that harbor bacteria-laden plaque. This colorless, sticky film can solidify as tartar, which has been likened to the barnacles on a ship and is similarly difficult to remove.

January 18, 2010 column BB 3

One of our basic philosophies at JOLLY SMILES is that our patients must recognize the need to take charge of their dental health and do what is necessary to prevent, arrest, or control their own dental conditions. We’ll work together as a team with you to achieve the long-term benefits that quality dentistry affords. From sound advice about periodontal care to routine well visits to major dental reconstructions and cosmetic dentistry, we’ve got your dental needs covered. Our office is located at 102 Sleepy Hollow Drive, Suite 100. You can reach us at 302-378-3384. We accept most insurance plans.

P.S. Because periodontal disease has been linked with heart disease, paying close attention to the health of your gums may help preserve your heart as well.

SEEING RED!

DENTAL BB 2

A dentist may be likely to provide a natural redhead with a bit more attention than blondes and brunettes might receive. The reason behind this special treatment rests with the fact that redheads seem to have a genetic disposition to be nervous about visiting the dentist. In fact, research in recent years shows that redheads tend to be less responsive to local pain-blockers and may require larger doses of Novocain to get the same amount of relief that a blonde or brunette would get with regular doses. More recent research shows that the red hair that results from variations in the melanocortin-1 receptor (MC1R) gene, which helps determine pigmentation, may also play a role in processing pain, anxiety, and fear.

P.S. Patients with any fear- or anxiety-related issues are strongly urged to speak with the dentist about their concerns and work out a treatment plan that addresses and allays those worries.

INCORRECTLY PEGGED FROM THE BEGINNING

DENTAL BB 1

So-called “peg laterals” are small, cone-shaped teeth that develop on either side on the top two front teeth. These undersized teeth appear to be “baby teeth” surrounded by larger permanent teeth. As a result, the smile looks imbalanced. Fortunately, the dentist can restore peg laterals to the size of normally developed incisors by building them up with composite material. This tooth-colored resin restoration requires no anesthetic and can be accomplished in one visit. After the dentist acid-etches the tooth’s enamel so it can properly accept the resin, layer upon layer of resin is applied to the tooth. After the composite material cures properly, the augmented tooth is shaped and polished. The result compares favorably with surrounding natural teeth.

January 4, 2010 column BB 1

Composite resin restorations and other advances in cosmetic dentistry make it harder than ever to tell the difference between what’s natural and what’s not. They also make it easier than ever to smile with confidence. This column is prepared as a public service to help our Middletown family and friends become aware of and better understand modern dentistry and the power of a healthy smile. If you would like more information on any of our topics or an appointment, please call us at JOLLY SMILES, 302-378-3384. Located at 102 Sleepy Hollow Drive, Suite 100, evening and Saturday hours are available.

P.S. A peg lateral incisor may be augmented with a crown.

WHAT SENIOR PATIENTS CAN EXPECT

DENTAL AA 60

While seniors can expect some age-related changes in their teeth and gums, certain changes should not be overlooked. For example, it’s natural for teeth to darken and show signs of wear as we age. Aging gums are also likely to recede, which has given rise to the phrase “long in the tooth.” As a consequence, dental cavities closer to the root of the tooth are more likely to occur. However, neither tooth decay nor tooth loss should be accepted as a normal part of the aging process. Bleeding gums is a sign of gum disease, which is the leading cause of tooth loss among seniors and should be quickly addressed. With proper treatment, seniors can expect to retain their teeth.

December 21, 2009 column AA 60

Regardless of age, everyone can benefit from preventive dental care. Time and again, experience proves it is to your advantage to take time for your oral health regardless of physical health and no matter how busy your personal or professional schedule. A few minutes spent each day on your teeth can save on future dental visits. Nothing is more economical than regular dental checkups, so please let us at JOLLY SMILES help you take care of your dental health. Please call 302-378-3384 for an appointment at 102 Sleepy Hollow Drive #100. Evening and Saturday hours are available.

P.S. Dry mouth is another common problem among seniors, which can lead to tooth decay and should be brought to the attention of the dentist.

CLOSING THE GAP

DENTAL AA 59

Gaps between the upper two front teeth may appear for the simple reason that there is more room than necessary to accommodate all of the teeth. Otherwise, it may be that the muscle attachment (frenum) that is found between the upper lip and the upper dental arch (in the area of the two front teeth) is too large or fibrous. If so, the front teeth may splay. A simple surgical procedure may be all that is necessary in this case to relieve the pressure exerted by the frenum. After that, the gap between the two front teeth may be closed using a bonding procedure that builds up tooth material on both sides of the space.

December 7, 2009 column AA 59

If you’re not happy with your smile, we hope you won’t be shy about examining the many options available today in cosmetic dentistry like bonding, Sapphire whitening, porcelain veneers, and reshaping. Our interest in you extends beyond just fixing your teeth—we want you to feel totally unselfconscious about your mouth so that you will have every reason to break into a smile. Call us for an appointment to discuss your particular dental needs at JOLLY SMILES, 302-378-3384. New patients are always welcome at 102 Sleepy Hollow Drive #100. Most insurance plans are accepted.

P.S. A frenectomy is a surgical procedure in which the frenum is cut away from the gum so that its attachment will be further away from the teeth.

THE ACID TEST

DENTAL AA 58

Dentists are observing high levels of dental erosion among patients, which is leaving their teeth cracked, discolored, and feeling more sensitive. These symptoms are due to loss of the teeth’s protective enamel coating that is being worn away under an assault from the acids found in a variety of beverages. The primary culprit is phosphoric acid, a primary flavoring agent in soft drinks, especially colas. In fact, phosphoric acid is the substance that dentists use to etch the surfaces of teeth that are about to receive the bonding agents used for patching and repairs. Sugar found in many soft drinks also plays a role in tooth erosion by feeding the bacteria in plaque that eat away at teeth.

P.S. To counter the problem of acid-induced dental erosion, dentists suggest limiting soft drink, sport drink, and fruit juice intake, along with increased use of fluoride, which builds up tooth enamel.

DO MEN BRUSH AS OFTEN AS WOMEN?

DENTAL AA 57

When the American Dental Association recently conducted a national survey, they found that men and women differ when it comes to taking care of their teeth and gums. According to the nationally representative survey of 1,000 Americans ages 18 and older, 86 percent of women brush their teeth twice or more a day while only 66 percent of men make the same effort to keep their teeth clean. As far as flossing is concerned, there is room for improvement for both genders. Slightly less than half of those surveyed (49%) indicated that they floss their teeth once a day or more. One out of three people surveyed think that bleeding gums after brushing their teeth is normal—but it is not.

P.S. According to the survey mentioned above, women replace toothbrushes every three to four months on average while men retain theirs an average of five months.

NEW WAY TO END RECESSION

DENTAL AA 56

A promising new treatment may provide dental patients with another option for treating receding gums. The traditional treatment for this problem involves surgically grafting tissue from the roof of the patient’s mouth and transplanting it onto the patient’s receding gums. The alternate procedure, known as “guided tissue regeneration” (GTR), begins with drawing blood from the patient, from which blood cells known as platelets are retrieved. The value of these platelets to dental researchers rests with the fact that they are rich in proteins called “growth factors,” which promote tissue repair and wound healing. Researchers found that soaking a collagen membrane with the platelets, then suturing it over the exposed tooth root, provided results comparable to the grafting method.

SUGAR SUBSTITUTE AS CAVITY FIGHTER

DENTAL AA 55

Approved for use in food in the United States since 1963, the sugar substitute xylitol has already been proven to have decay-fighting ability as an ingredient in chewing gum among school-age children with permanent teeth. More recently, researchers have found that much younger children, who received eight grams of xylitol daily in syrup form, also experienced a significant cavity-fighting benefit. In fact, the baby teeth of children between the ages of 9 and 15 months who received the daily dose of xylitol experienced up to a 70 percent reduction in tooth decay. However, researchers were quick to point out that this is not to be construed as a silver-bullet approach, as fighting tooth decay should be a multi-pronged effort.

November 23, 2009 column AA 55

To maintain good dental hygiene and a healthy smile, it is essential to take the necessary steps to prevent both tooth decay and periodontal disease. This includes a healthy diet, conscientious brushing and flossing, and periodic professional cleanings and exams. Research repeatedly demonstrates that those people who own better dental health live longer, stay healthier longer, and enjoy a better life. If you’d like to maximize your oral health, thereby enhancing every day of your life, why not call us at JOLLY SMILES, 302-378-3384, for an appointment? We’re located at 102 Sleepy Hollow Drive #100.

P.S. Xylitol may one day be applied to children’s teeth much in the way that fluoride now is to prevent tooth decay.

PROFESSIONAL TOOTH WHITENING

DENTAL AA 54

Professional tooth whitening ensures patients of the safest and most successful outcomes. The American Dental Association recommends the use of a carbamide peroxide solution (available in 10%, 16%, and 22% concentrations) in a custom-fit tray for use at home. These customized mouth guards help ensure that the tooth-whitening solution remains in contact with the teeth for maximum whitening while minimizing the potential for irritating gums and other soft tissues in the mouth. Professional tooth-whitening also provides the benefit of consultation with the dentist in the event that the patient experiences undue tooth sensitivity. If so, the dentist can provide a tooth whitener of lower concentration or a desensitizing gel for use immediately preceding or after the use of the tooth whitener.

November 16, 2009 column AA 54

If you’ve looked at yourself in the mirror and wished you liked your smile more, why not make a change for the better? Teeth can be whitened and their appearance improved by several methods of cosmetic dentistry, including Sapphire whitening. In fact, it is truly amazing what can be accomplished in a short time that can enhance your appearance for the rest of your life. Comprehensive, quality, gentle dental care for patients of all ages is available here at JOLLY SMILES, 102 Sleepy Hollow Drive #100. And new patients are always welcome—call 302-378-3384 for an appointment.

P.S. Dentists can also prescribe tooth-whitening gels in the proper doses so that patients do not feel compelled to overuse the product.

PREGNANCY AND TOOTH LOSS

DENTAL AA 53

The old adage “for every child she has, a mother loses a tooth” has largely been disparaged by dentists as outmoded. However, recent research suggests that there is a bit of truth behind this old wives’ tale. According to the study of 2,635 women, those with no children had lost one to two teeth while women with children tended to lose more. Looking specifically at women in the 35- to 49-years age range, researchers found that those with just one child had lost an average of three teeth, and women with two children lost an average of four. The number of teeth lost increased to five for women with three children and seven for those with four or more children.

November 9, 2009 column AA 53

Although gingivitis and periodontal disease are caused by plaque, other factors can increase the risk, severity, and speed of their development. These factors include pregnancy, medications, smoking, impacted food, poorly fitting bridges, defective restorations, poor diet, disease, and bad habits You can still achieve good oral health, though, by following your dentist’s advice and practicing preventive hygiene. If you are new to the Middletown area and haven’t found the right dentist yet for your family, we invite you to call our office at JOLLY SMILES, 302-378-3384, to set up an appointment at 102 Sleepy Hollow Drive #100.

P.S. It’s not that tooth loss inevitably comes with pregnancy, but that pregnancy poses challenges (such as susceptibility to gingivitis) that women must adequately address in order to maintain good oral health.

GOOD FOR TEETH, GOOD FOR OVERALL HEALTH

DENTAL AA 52

What you eat may affect not only your oral health, it may also similarly influence the health of your entire body. Researchers have been looking at the so- called “fermentable carbohydrates” that form the foundation of the modern diet. Dentists have been telling us for years that sugars and starchy foods increase the risk of tooth decay and gum disease. These same foods are also likely linked to the development of chronic diseases such as heart disease and diabetes. Thus, tooth decay and bleeding gums may raise a red flag that a patient may be at risk for other chronic health problems. What is good for the teeth and gums is also good for the rest of the body.

P.S. Studies show that gum disease develops within days of a person having increased his or her sugar intake.

NERVOUS ABOUT SCHEDULING AN APPOINTMENT?

DENTAL AA 51

Many people experience dental anxiety to the point where they deny themselves the treatment they need to maintain good oral health. If the thought of sitting in a dentist’s chair makes you nervous, you are encouraged to call the office and schedule a consultation. During your visit, the dentist can discuss the various methods that can be utilized to make your visits as comfortable as possible. To begin with, sedation treatment is available, which can render the whole experience a pleasant one for patients. Beyond that, there is needle-free anesthesia and lasers that can take the place of drills. Both are designed to render treatment pain-free. Most importantly, the dentist can establish a truthful, trusting relationship that assures confidence.

IMITATING NATURE

DENTAL AA 50

It’s hardly surprising that dental implants are among the most highly successful procedures in dentistry. Dentists draw on nature’s own design to achieve the most structurally sound, natural looking, and best fitting tooth replacements. This entails embedding a titanium implant in the patient’s jaw, which integrates with the surrounding bone to create a foundation as solid as a natural tooth root. The implant is then ready to accept the abutment and prosthetic tooth, which are attached as firmly as a natural tooth. As a result, dental implant patients experience a degree of confidence that cannot be matched by any other tooth replacement procedure. The only thing that compares with a dental implant is a natural tooth.

October 12, 2009 column AA 50

Wouldn’t you like to bite into a sandwich knowing your dentures won’t slip? Go to sleep each night without removing your teeth? Smile with confidence? Implants may be the answer for you. Implants are an exciting tooth-replacement option that has changed the lives of many people. In fact, more than a million have been placed in North America alone. Are they for you? We welcome your call at JOLLY SMILES for a consultation about your future smiles. You can reach us at 302-378-3384, 102 Sleepy Hollow Drive #100. Most insurance plans are accepted.

P.S. Implants can be used to hold a bridge firmly in place.

GOING THROUGH A ROUGH PATCH?

DENTAL AA 49

A comprehensive oral exam involves both an examination of the teeth and a close inspection of the soft tissues of the mouth. One condition the dentist or hygienist will be looking for are “leukoplakia.” This generic term applies to any type of white patch that occurs inside the mouth that cannot be easily scraped off. In some cases, these patches represent calluses or some other benign condition. In others, leukoplakia are associated with smoking and regular, heavy alcohol use. If so, they are considered to be a precancerous condition. Smokers and heavy drinkers may have tongues with white or brown coatings that are common manifestations of leukoplakia. Most cases are not serious, but others could be life-threatening if left untreated.

P.S. Leukoplakia is usually harmless, and lesions generally clear in a few weeks to a month after the source of irritation is removed.

RIDGE AUGMENTATION

DENTAL AA 48

One potential consequence of tooth loss is that the bone that once held the tooth in place will disintegrate, resulting in the collapse of the overlying gum tissue. The gum will then exhibit an indentation, cleft, or unevenness at the gum margins. Not only can the gum concavity look unnatural and unappealing, the indentation can compromise the look and fit of the replacement tooth. To properly address this problem, the dentist can perform a cosmetic periodontal procedure called “ridge augmentation,” which involves surgical implementation of grafted bone and/or gum tissue. The results of this procedure are very predictable and long-lasting. In order for teeth to look healthy and natural, they must be properly framed by gums with natural contours.

P.S. Ridge augmentation can be performed in the dentist’s office using local anesthetic.

JUST SCRAPING BY

DENTAL AA 47

Without proper treatment, older patients with periodontal disease face the potential for tooth loss. The object of treatment for periodontal (gum) disease is to remove plaque; calculus; and other products associated with plaque accumulation that have the potential to destroy gums, bone, and supporting structures around the teeth. To this end, the dentist and/or hygienist may scale (scrape) teeth to remove sticky plaque and calculus (hardened plaque) from exposed tooth surfaces. Accessible root surfaces may also be planed to make them smooth. As a result of these efforts, bacteria are removed and find it difficult to reattach to tooth surfaces. This, in turn, fosters the reattachment of connective tissue to the tooth, which serves to hold teeth in place.

September 28, 2009 column AA 47

Prevention is the keystone of good dental health no matter what our age. Through regular visits with the hygienist and dentist, plus good habits like proper brushing and flossing at home, we have more control than ever before over the health of our teeth and gums. October is National Dental Hygiene Month, dedicated to increasing public awareness of the importance of oral health and the dental hygienist’s role as the preventive professional. Come celebrate with us and have your teeth cleaned at JOLLY SMILES, 102 Sleepy Hollow Drive #100. Please call 302-378-3384 for an appointment.

ORAL CANCER SCREENING

DENTAL AA 46

While some regard the incidence of oral cancer as rare, the Oral Cancer Foundation reports that about 100 people in the United States are newly diagnosed with this form of cancer each day. Fortunately, early diagnosis and treatment greatly increase the chances of survival. With this in mind, dentists recommend oral cancer screenings once a year for all patients, twice a year for smokers. Aside from a visual exam, oral cancer screenings also involve the use of a device known as a VELscope, a handheld instrument that emits a blue light that reveals any abnormalities that may exist beneath the surface of the gums, tongue, and other soft tissues of the mouth. The painless exam takes about two minutes.

September 14, 2009 column AA 46

Your total health is always our concern at JOLLY SMILES. That’s why all new patient exams are so thorough, so we can establish a baseline against which we can measure your future oral profile and progress. IF PATIENTS HAD A WAND

DENTAL AA 45

If dental patients could wave a magic wand, many might wish that the discomfort associated with an injection of local anesthetic would disappear. In fact, it is not the prick of the needle so much as the pressure and volume of fluid being injected into the gums and soft tissues that some find so uncomfortable. Understanding this phenomenon can help patients realize why the computer-controlled local anesthesia injection system known as The Wand can provide a more comfortable and effective anesthetic delivery. The Wand utilizes a microprocessor to automatically compensate for different tissue densities and deliver anesthetic at a constant pressure and volume that is typically below the pain threshold of a patient, resulting in greater comfort.

P.S. The Wand provides a flow of anesthetic ahead of the needle to numb the site of insertion to minimize patient discomfort.

AT THE ROOT OF TOOTH PAIN

DENTAL AA 44

In cases in which tooth decay is left untreated, bacteria may infect the tooth pulp and lead to an abscess. Symptoms include aching or throbbing pain in the tooth, sensitivity to hot or cold, chewing pain, and fever. At this point, endodontic (root canal) treatment is indicated. This involves removal of the nerve and vascular tissue (pulp) from the root and pulp chamber, as well as any associated decayed tooth structure. This allows the tooth to be spared from extraction. After the inside of the tooth is carefully shaped and cleaned, it is filled and sealed, awaiting a crown. Preserving the tooth root in this fashion helps avert the bone loss that often accompanies loss of the tooth root.

P.S. The pain associated with root canal therapy is due exclusively to the pain caused by the abscess and not the treatment, which removes the cause of the pain.

GETTING YOUR TEETH INTO SHAPE

DENTAL AA 43

It only takes an irregularity or defect as seemingly slight as a small chip or worn edge on a front tooth to significantly detract from a smile. Fortunately, the dentist may be able to restore the smile to its former beauty through tooth contouring and reshaping. This cosmetic procedure involves changing the size or shape of the irregular tooth, thereby restoring it to its former appearance. The procedure typically entails one office visit, during which the dentist uses various tools to carefully polish small areas of the surface enamel. Then, the edges of the newly reshaped tooth are smoothed. After contouring, the treated tooth fits in better with adjacent teeth in terms of both shape and size.

August 31, 2009 column AA 43

Don’t let the presence of chipped, discolored, stained, or otherwise less than perfect teeth keep you from smiling. There are many ways your dentist can use cosmetic dentistry to turn your smile into one you’ll be proud to show. THE HYGIENIST LENDS A HAND

DENTAL AA 42

During a regular checkup, the hygienist can often tell whether you are right-handed or left-handed simply by identifying the location of plaque build-up on your teeth. Depending on which hand you use to brush your teeth, some areas are likely to be brushed more rigorously than others. In addition, most people tend to brush the outer surfaces of their teeth more completely than the inner (lingual) sides. As a result, the inside surfaces of the lower front teeth are the ones that accumulate the most plaque and tartar. The gums in these areas are more likely to be susceptible to gum disease. To counter this problem, it is suggested that the insides of the lower front teeth be brushed first.

P.S. For the best results, switch from your manual toothbrush to a rotary electric toothbrush, which is similar to the one that dental hygienists use for professional cleanings.

OUT OF BOTH SIDES OF THE MOUTH

DENTAL AA 41

One problem that can arise from the failure to replace lost teeth is a condition known as “angular cheilitis,” inflammation of the corners of the mouth caused primarily by a reduction in the vertical dimension of the bite. As the lips overlap more at the corners of the mouth and leak saliva, the stage is set for irritation and inflammation. While the fungal and bacterial growth that leads to skin irritation and cracking may be stemmed with medications such as anti-fungal, antibiotic, and steroid creams, the root cause of the problem must be addressed. Treatment usually entails replacement of lost teeth. If dentures are already in place, adjustment of denture fit and/or better adherence to denture-cleaning guidelines may be required.

August 17, 2009 column AA 41

We want you to be comfortable with us no matter what your oral concerns. We try to make all our patients’ visits as relaxing as possible. If you are hesitant about visiting the dentist, for an oral inflammation, dry mouth, bad breath, or other oral issues, we invite you to call us at JOLLY SMILES, and find out how satisfying and rewarding a visit to the dentist can be. PRESCHOOLERS’ RISING CAVITY RATE

DENTAL AA 40

A recent report from the U.S. Centers for Disease Control and Prevention reveals that the cavity rate among preschool children is rising. According to the most recently available data, 28 percent of children between the ages of two and five had decay in primary teeth during a recent five-year period. Much of the blame for increased tooth decay among very young children is placed on reduced use of tap water, which is likely to contain fluoride. Bottled water, on the other hand, which has gained widespread popularity in recent years, contains no enamel- protecting fluoride. Parents are also advised not to put babies to bed with bottles containing juice or formula, which contain sugars that contribute to tooth decay.

August 10, 2009 column AA 40

Taught early enough, healthy habits turn into healthy lifestyles. Begin with a healthy diet, as well as proper brushing and flossing techniques. Continue with regular professional care and cleanings for a healthy smile that will last a lifetime. A RELATIVELY EASY REPAIR

DENTAL AA 39

Teeth that are chipped, discolored, or irregularly shaped lend themselves to a relatively easy repair known as “tooth bonding.” This cosmetic technique involves the use of a composite resin that the dentist uses to coat and reshape tooth surfaces, much in the way that a sculptor might use sculpting clay. Bonding can be used to lighten stains (by covering them), close minor gaps between teeth, and correct moderately misshapen teeth. Once the resin is applied to the tooth surface, it is hardened with a high-intensity light. Then, the dentist sculpts the hardened resin and polishes it. The resultant restoration matches surrounding teeth in terms of both color and form. In nearly all cases, tooth bonding involves a single visit.

August 3, 2009 column AA 39

In addition to the way other people see you, the many methods of cosmetic dentistry can greatly improve the way you see yourself. BRUSH AWAY HEART ATTACKS?

DENTAL AA 38

According to the most recent piece of research that links oral hygiene with overall health, people may be able to lower their heart attack risk by brushing their teeth more often. It seems that researchers have found that people with the most germs in their mouths are the most likely to experience heart attacks. While Tannerella forsynthesis and Prevotella intermedia were found to be the most common bacteria in the mouths of heart attack victims, the most common indicator of heart attack risk was the total amount of bacteria in heart patients’ mouths. With this in mind, researchers are underscoring the importance of tooth-brushing and regular dental exams in minimizing heart attack risk.

Keeping up with regular dental checkups will give your dentist the opportunity to keep an eye on your health as well as any possible areas of developing gum disease. We’re located at 102 Sleepy  Hollow Drive #100.

P.S. Several studies have linked gum disease with heart disease, giving rise to the possibility that bacteria may set off general inflammation that, in turn, causes blood to clot.

UNWAVERING COMMITMENT TO HYGIENE

DENTAL AA 37

As public attention focuses on a number of transmitted diseases, this office would like to affirm our unwavering commitment to stringent guidelines set forth by the Occupational Safety and Health Administration, which ensure protection from infectious diseases. These measures include steam autoclaving all instruments and hand pieces after every use. Disposable items are used whenever appropriate. Dentists and staff wear protective eyeglasses and disposable gloves during treatment. All areas that are touched during the procedures are covered with a plastic wrap and changed after each patient to eliminate cross contamination. All surfaces, chairs, lights, and units are thoroughly disinfected before and after every patient’s use. Attention to these and other measures ensures our patients’ (and our staff’s) health and safety.

JOLLY SMILES provides the highest quality of preventive care so that you’ll spend less time in treatment and more time enjoying your healthy smile. We offer complete general dental care, from cleanings to full mouth reconstruction to repair of any damage done by neglect or trauma. We also provide cosmetic dentistry here at 102 Sleepy  Hollow Drive #100, for the sunniest smiles possible. Call us at 302-378-3384 for an appointment.

P.S. An autoclave is an instrument-cleaning device that utilizes steam under pressure to kill pathogens; ultrasonic cleaners do so with sound-wave technology.

OBESITY INCREASES PERIODONTAL-DISEASE RISK

DENTAL AA 36

In its mild form, gingivitis (gum disease) may only lead to bleeding gums. However, if not treated, gingivitis can progress to a more severe stage, periodontitis, in which bacteria-laden plaque provokes an inflammatory response in gums. To avoid the potential of infection and tooth loss, it is important to identify causes of gum disease, as well as to diagnose and treat it. With this in mind, researchers have recently found that obese adults are at higher risk of gingivitis than people of normal weight. In fact, over the course of the 16-year study, obesity was found to be associated with a 29 percent increase of periodontal disease. The common denominator is thought to be inflammation.

P.S. In addition to the obesity link with gum disease in the study mentioned above, a waist measurement of 40 inches or more in men was linked to a higher risk of heart disease.

ANTIBIOTICS BEFORE DENTAL PROCEDURES?

Dental AA 35

Patients with hip prostheses may have heard that it is advisable to take antibiotics prior to undergoing tooth cleaning, scaling, and other procedures to address the potential release of bacteria into the bloodstream caused by these treatments. For people with replacement joints, the concern is that bacteria in the bloodstream (“bacteremia”) could cause an infection by attaching to the artificial joint or surrounding tissues. However, such infections are quite rare. In fact, the American Dental Association recommends against the use of antibiotics prior to dental procedures with the sole intention of preventing infections in artificial joints. Exceptions to this recommendation involve patients who are very susceptible to infection or are having a procedure that carries a higher risk of bacteremia.

P.S. Pre- treatment antibiotics may be indicated for immune-compromised patients and those who have inflammatory arthritis, type 1 diabetes, or hemophilia.

A SMILE AS YOUTHFUL AS YOUR FACE

Dental AA 34

Interestingly, the American Dental Association notes that teeth were “designed” to last only about 30 years, which means they may need a little help in keeping up appearances. Aside from the usual problems of decay and discoloration, older adults face other potential problems that can undercut the youthfulness of their smiles. For instance, the incisors (or canines) wear down as the years pass, leaving less enamel exposed below the upper lip when a person smiles. Moreover, teeth drift toward the center of the mouth as the bone and tissues that hold them in place weaken with age. As a result, teeth can appear crowded and unevenly distributed. Fortunately, there are cosmetic procedures available that can help restore a smile’s youthfulness.

July 6, 2009 column AA 34

Today, there is no need to worry about an imperfect smile—it can be repaired using any number of the methods now available.  And aside from the obvious aesthetic benefits of cosmetic dentistry, it can often prevent more serious dental health problems down the road. For more information about the many advantages of cosmetic dentistry, or for an appointment for family dental care, please call us at JOLLY SMILES, 302-378-3384. Our office is located  at 102 Sleepy  Hollow Drive #100, where we use the Sapphire whitening system for the brightest smiles possible.  Most insurance plans are accepted.

P.S. If a lost tooth is not replaced with either an implant or a bridge, adjacent teeth will move to fill the space, disrupting the function and appearance of the remaining teeth.
