This substance, which is found in a variety of fruits and vegetables, is just about as sweet as sucrose (ordinary sugar) but has fewer calories and no aftertaste. More importantly, xylitol prevents decay-causing bacteria from sticking to teeth, thereby reducing plaque formation. TREATING MOM’S GUMS SAFE FOR BABY

As research continues to uncover important links between the health of the gums and overall health, it is becoming increasingly clear that gum inflammation (gingivitis) should be prevented or treated early. This holds especially true for pregnant women, who undergo hormonal changes that render them more susceptible to gum disease. While standard antibiotic-based therapy must be avoided because it stains babies’ teeth, aggressive tooth-cleaning (“scaling and planing”) is also often avoided in the belief that it could lead to bacteria entering the blood stream and harming the fetus. However, recent research finds this fear to be unfounded. Testing of toddlers whose mothers were treated with aggressive cleaning while pregnant showed no signs of developmental harm. Prevention is always preferred, however.

It may be glib to say that such news is all part of the “daily grind,” but these stress-inducing events are no laughing matter for those suffering from “bruxism.” This term refers to the grinding and clenching of teeth that many stressed-out individuals endure while they sleep. According to the Academy of General Dentistry, about one in three people suffers from bruxism, particularly aggressive, competitive, and hurried individuals. As their anxiety mounts, teeth and prosthetic crowns become more susceptible to fracture. To protect their teeth, bruxers are encouraged to see the dentist about wearing protective night guards.

The FDA recently issued a safety announcement that warns parents against using benzocaine teething products in children under the age of two years. The use of these products (including Orajel, Baby Orajel, Orabase, and Anbesol) can lead to a serious condition known as “methemoglobinemia,” which involves the inability of the oxygen-carrying protein in red blood cells to transport oxygen to the body’s cells and tissues. Fortunately, there are a number of alternative treatments to benzocaine teething gels, including chilled rubber rings that babies can chew on to soothe their teething discomfort. Solid frozen rings, on the other hand, should be avoided since they can cause injury to baby’s tender gums. Parents can also massage their baby’s gums with clean fingertips

DOES PERIODONTITIS INCREASE CANCER RISK?

As evidence mounts linking gum disease to other serious conditions such as heart disease and even premature birth among pregnant women, dentists and hygienists have been highlighting the importance of maintaining the health of the gums. Not only is it critical to brush and floss twice daily at home, patients are strongly encouraged to undergo professional dental cleanings to prevent or alleviate inflammation of the gums (“gingivitis”). Otherwise, if left untreated, gum disease may lead to a more serious condition, chronic periodontitis. This advanced form of gum disease can result in bone and tooth loss. In addition, research indicates that periodontitis also increases the risk of developing head and mouth cancers, especially in the mouth and throat.

While we have all been told that eating too much candy is bad for our teeth, relatively few patients are aware that seemingly healthy and nutritious foods such as raisins and whole wheat bread also pose potential problems. The fact is that all carbohydrates break down into simple sugars, which then are converted into “plaque” by bacteria in the mouth. It is this sticky residue that is primarily responsible for tooth decay and gum disease. This risk is increased when carbohydrate-based foods such as raisins and whole-grain breads adhere to tooth surfaces and collect below the gum line. At this point, they can pose nearly as much of a danger to oral health as caramel in candy bars

ROOT CAUSE OF PAIN

When a deep cavity or crack causes infection that damages a tooth’s pulp (which contains blood vessels, nerves, and connective tissue), severe pain results. If left untreated, the damage can spread to the surrounding bone, causing further problems. For this reason alone, patients with an infection in the pulp of a tooth need to undergo endodontic treatment that involves drilling to the core of the tooth; removing the damaged pulp; and then cleaning, shaping, filling, and sealing the tooth. A “root canal” procedure derives its name from the fact that the pulp branches down each root through canals that stop just short of the bottom tip of the tooth. Endodontic treatment preserves teeth and circumvents the need to extract them.

ALL-PORCELAIN CROWNS

When it comes to choice of material for replacement crowns, porcelain provides the most natural appearance. Much like natural teeth, translucent porcelain does not have a monochromatic appearance. Instead, it displays color variation that results from the internal play of light. Porcelain’s texture also plays a role in imparting an organic look to replacement teeth by breaking up light reflections. The first all-ceramic crowns introduced over a century ago were the porcelain-jacket type. While they offered superior cosmetic appearance, they were not thought to be strong enough to be used on back (chewing) teeth. More recently, however, proprietary materials and processes have been developed for fabricating all-ceramic crowns out of solid blocks of material that combine excellent strength and beauty.
P.S. All-ceramic crowns tend to be more natural looking than porcelain-fused-to-metal (PFM) dental crowns, and they give no indication of the metal edge of their substructure like PFM crowns do.

CHLORINATED POOL WATER’S POTENTIAL RISK

There is little question that swimming is a healthy activity. However, those who frequently swim or spend prolonged periods of time in chlorinated pools should be aware of the potential for tooth enamel damage. While chlorine is added to pool water to provide swimmers protection against bacteria, using too much chlorine can lower pH levels in water to the point where it can erode tooth enamel. Curiously, brushing teeth after they are exposed to chlorinated pool water with low pH (acidic) may do more harm than good. Abrading the weakened tooth enamel with a brush may only hasten the erosion. Rinsing with a fluoride product may be preferable. Consultation with the dentist can provide a proper diagnosis and treatment.
P.S. Because most sodas and sport drinks contain acids, it may benefit teeth to drink them through a straw.

PLAQUE DETECTION

Although your teeth may seem clean, they still may be encrusted with
germ-laden plaque, especially between teeth and at the gum line. To
detect just how effective tooth brushing and flossing are in removing
the sticky substance that causes both tooth decay and gum disease,
patients can undergo a plaque detection test either at home or in the
dental office. The home version of the test utilizes special tablets
that contain red dye. Once the tablet is chewed and mixed with saliva,
the dye is swished around in the mouth for about 30 seconds. After a
water rinse, the teeth are examined. Any pink residue on the teeth
indicates remaining plaque, which poses a threat to teeth and gums.

is one of a number of amino acids that serve both as “building blocks”
of proteins and as intermediates in metabolism. What has recently come
to the attention of dentists is the fact that, in addition to amino
acids’ role as the building blocks of muscles, tendons, organs, glands,
nails, and hair, proline has been identified as the amino acid
responsible for making teeth strong and resilient. Researchers have
found proline to be repeated at the center of proteins found in tooth
enamel, which is the hardest substance found in the human body. This
new finding may lay a foundation for future research that helps
dentists replace damaged parts of the outer-tooth layer with healthy
layers of new enamel.

Other animals, such as frogs, do not have proline repeats as long as
those found in humans’ teeth, which causes frogs’ teeth to be softer
than those of humans.

TOOTH RESHAPING

minor defects such as small chips, shallow pits, and worn edges detract
from a smile, “dental contouring” (or “tooth reshaping”) provides a
quick, safe, and easy fix. This cosmetic procedure may also help
correct crooked and overlapping teeth and may even be a substitute for
braces under certain circumstances. The procedure involves using a
sanding drill to artfully and gradually remove minute amounts of tooth
enamel in order to attain the desired shape or eliminate any unwanted
characteristic. The dentist will likely mark the teeth with a pencil
prior to the procedure to outline the work. As the teeth are sculpted,
imperfections are eliminated or minimized. Finally, the teeth are
smoothed and polished. Anesthetic is rarely needed.

PIERCING INTELLIGENCE

The
American Dental Association opposes tongue piercing due to potential
complications arising from puncturing the tongue and inserting a
decorative post. Perhaps the most serious of such complications is the
risk of infection, one source of which is thin layers of bacteria
(called biofilms) that coat piercings and attract germs. Interestingly,
a recent study found that stainless steel and titanium studs are likely
to collect more bacteria than plastic studs. An examination of pierced
tongues showed that stainless steel studs harbored the most bacteria
(including Staphylococcus aureus and Haemophilus influenza, which can
cause body-wide infections). The titanium studs collected bacteria to a
lesser extent, while plastic studs accumulated significantly fewer
bacteria. However, any tongue piercing poses an unnecessary risk.

only does gum disease afflict a large portion of the adult population
and pose the threat of tooth loss, this inflammatory condition has also
been linked with an increased risk of heart disease, stroke, and other
serious conditions. With this in mind, patients should be scrupulous
about oral care at home and scheduling regular dental appointments. It
may also pay to increase consumption of omega-3 fatty acids found in
fish. New research indicates that omega-3s significantly decrease the
risk of gum disease. People consuming the most DHA (one of the
principal omega-3 fatty acids found in fish) were 22 percent less
likely to suffer periodontitis; EPA (the other important fish-oil
omega-3) also lowered the risk, but not as much.

HEROES AND VILLAINS

One
villain responsible for tooth decay is the bacterium streptococcus
mutans, which breaks down sugar and produces the sticky molecules
(glucans) that are responsible for allowing bacteria to cling to our
teeth. This villainous bacterium also produces an enamel-eroding acid
responsible for cavities. The heroes that counter these effects are
called “polyphenols,” which are found in the fermented stems, seeds,
and grape skins left over from wine production. These compounds, which
are also found in cranberries, block streptococcus mutans’ ability to
produce glucans. As helpful as polyphenols are, however, no one is
suggesting that we drink wine or sugary cranberry drinks to prevent
tooth decay. Instead, scientists hope to isolate these compounds and
put them in toothpaste and mouthwashes.

DIABETICS AND GUM DISEASE

Diabetics should be conscientious about visiting the dentist as often
as recommended. According to a recent review of data from seven
studies, patients with both diabetes and gum disease who were treated
for their periodontal conditions over a four-month period displayed
better glucose control than diabetic patients whose gum disease was
left untreated. Periodontal disease, a bacterial infection of the gums
that induces redness and inflammation, can be effectively treated with
antibiotics and plaque removal by the hygienist. Plaque is the

TREATMENT MAY REDUCE PREMATURITY RISK

Pregnant women should make visits to the dentist part of their overall
prenatal-care regimen. According to new research, doing so may reduce
the risk of premature birth, which is associated with periodontal (gum)
disease. This new study may alter the way that dentists treat gum
disease in pregnant women. The bacterial infection associated with gum
disease breaks down gum tissue and leaves it chronically inflamed. This
is a particular problem during pregnancy because hormonal changes
appear to make pregnant women more susceptible to periodontal disease.
Yet, standard tetracycline-based therapy used to treat it is considered
to pose a risk to the baby. However, aggressive non-drug tooth cleaning
(root scaling and planing) is safe and reduces the risk of premature
birth.

“Teething” is the term used to describe the emergence of a baby’s first
(primary) teeth through his or her gums. As might be imagined, this can
be a painful experience for children between the ages of four and seven
months. If your baby’s teeth emerge later, bear in mind that there is a
great deal of variability to eruption times. Aside from drooling,
crying, and other signs of irritability, a teething baby may have a
low-grade temperature of up to 100 degrees Fahrenheit. Any temperature
above this level should not be considered to be associated with
teething and should be brought to the attention of the pediatrician. To
allay the pain, try offering your child a frozen teething ring.

GETTING THE FULL TREATMENT

Fluoride treatments during regular professional cleanings make sense
for adults because they still get cavities. This is especially true at
the root surfaces and under crowns, which become exposed as adult gums
recede. Particularly susceptible to cavities are root surfaces because
they lose mineral faster than enamel does. With this in mind, higher
concentrations of fluoride provided by topical treatments may be needed
for adequate protection. It should also be noted that many adults take
certain medications and have medical conditions that cause dry mouth,

serious potential consequence of periodontitis (advanced inflammation
or infection of the gums) is tooth loss due to jaw-bone shrinkage and
damage. The latest effort to counter this threat comes in the form of a
drug called teriparatide, which has recently been proven to regrow bone
in jaws. According to one study, patients with tissue damage caused by
periodontitis developed nearly ten times more bone with teriparatide
treatment than those receiving daily placebo injections. The
significant gains in bone produced by the drug developed over a
relatively short six-week period. These improvements were sustained,
and even improved upon, over a succeeding 12-month period. This jaw-
regrowing ability may also prove useful for potential tooth implant
patients, who need more foundation bone.

and helps women stave off heart disease until they reach menopause. At
that point, as estrogen production wanes, women begin to catch up with
men in terms of heart disease risk. The good news is that older women
who receive regular dental care have a one-third less risk of
developing heart disease than women who don’t pay close attention to
their oral health. That is the conclusion reached by researchers who
analyzed the medical records of nearly 7,000 adults aged 44 to 88.
Although the study found no beneficial link between regular dental care
and heart disease prevention in men, women were found to benefit by
having fewer heart attacks and strokes.

harmful. Bathing teeth in a sugary liquid in this manner promotes tooth
decay. According to various studies, as many as 11% of pre-school
children have cavities caused by sucking on a bottle. If a bottle-fed
child’s mouth is not properly cleansed after sucking on a bottle by
lightly swabbing his or her teeth and gums with a water-dampened cloth,
plaque will accumulate to cause cavities. If your child must have a
bottle at bedtime, provide one filled with water.

easy fixes for this problem. To begin with, the dentist may reshape a
chipped tooth by removing small amounts of enamel (enamel recontouring)
to smooth and even out the tooth outline. Otherwise, bonding may be a
more suitable option. This tooth-restoration technique involves
painting a composite resin directly onto the tooth. Subsequent layers
can be added to fill in the space left by the chip. Finally, the bonded
portion of the tooth is sculpted to restore the tooth to its original
shape.

people think they are following the two-minute tooth-brushing regimen,
actual timing of their routines may prove otherwise. If necessary,
patients should use an old-fashioned egg timer to time themselves.
Flossing is also a must at least once a day because it reaches between
teeth where toothbrush bristles cannot go. Regarding regular visits to
the dentist, “two” visits a year is acceptable for most patients, while
visits every three to four months is preferable for those with bridges,
crowns, or a history of gum disease.

PREVALENCE OF GUM DISEASE HIGHER THAN ESTIMATED

incidence of gum disease in U.S. adults may have been underestimated by
as much as 50 percent. This surprising news comes from a pilot study
funded by the Centers for Disease Control and Prevention’s Division of
Oral Health, which conducted full-mouth, comprehensive, periodontal
examinations on more than 450 adults over age 35. When the findings
were compared with previous (partial-mouth) studies, it was found that
the prevalence of moderate to severe gum disease may have been
underestimated by 50 percent. This finding has serious implications
because periodontal (gum) disease is an inflammatory condition that not
only can lead to tooth loss, but it has been linked with other chronic
inflammatory diseases including diabetes, cardiovascular disease, and
rheumatoid arthritis.

only does the buildup of the sticky, bacteria-laden material known as
“plaque” increase the incidence of decayed tooth enamel, it also
promotes “gingivitis” (inflammation of the gums). When these sticky
deposits of bacteria, mucus, food particles, and other irritants
accumulate around the base of the teeth, they exert a toxic effect that
causes the gums to become irritated, red, and swollen, and to bleed
more easily. These symptoms are all signs of gum disease. Left
untreated, gingivitis can develop into the more serious condition
called “periodontitis,” inflammation of the membranes around the base
of the teeth, which can lead to erosion of the bone holding teeth in
place and resultant tooth loss. To avert these potential problems,
brush and floss daily.

GOING THROUGH A ROUGH PATCH

dentist. “Leukoplakia,” a thick, whitish patch that occurs on the
cheeks, gums, or tongue, is caused by irritation due to rough teeth or
rough surfaces on dentures, fillings, and crowns. It may also result
from smoking or other tobacco use (smoker’s keratosis). Because it can
progress to cancer, the dentist may want to take a biopsy. Patients
should also contact the dentist for an immediate examination if they
should notice a persistent sore or irritation that does not heal, color
changes (such as the development of red and/or white oral lesions), or
pain or tenderness anywhere in the mouth or lips.

BRIDGE BUILDING

“bridge” is a fixed dental restoration that is commonly used to replace
no more than four adjacent front teeth or two side teeth. Much like a
highway bridge, it is supported by two abutments, which are
structurally healthy teeth on either side of the gap. The span, or
“pontic,” is a metal framework attached to the abutments at each end
with the necessary number of attached artificial teeth. When properly
crafted, the artificial teeth and abutment crowns should match the
contour and color of natural teeth and appear to be growing out of the
gums. However, the pontic should barely touch the gums because exerting
unnecessary pressure on the gums and underlying bone can damage them.

the body really functions alone. The latest example of this
interdependency involves the finding that cardiovascular disease (and
coronary heart disease, in particular) is closely related to the number
of teeth that a person has left. According to recent research, a person
with fewer than ten of his or her own teeth has a seven times higher
mortality risk by coronary heart disease than a person of the same age
and of the same sex with more than 25 teeth left. The common
denominator between fewer teeth and heart disease is thought to be
chronic inflammation.

because they know that they are temporary, parents often do not regard
their children’s “baby teeth” with the same amount of respect they give
to the permanent teeth that replace them. As a result, parents usually
wait until their children’s primary teeth begin to appear before they
make an appointment for them to visit the dentist. In fact, a poll by
the American Academy of Pediatric Dentistry reveals that nearly three-
quarters of parents (70%) wait until their children are three years old
before scheduling a dental appointment. Dentists recommend that parents
schedule their child’s first dental exam by age one or within six
months of the eruption of the first baby tooth.

disease. If this percentage does not motivate everyone to floss daily,
it should be noted that failure to floss can also make a person look
older. Overgrowth of plaque creates gum inflammation that can lead to
gum recession and bone loss. In fact, the term for aging “long in the
tooth” derives from receding, uneven gumlines that make people look
older. On top of that, when gum disease leads to bone loss, lips and
cheeks cave in and wrinkles form. This puckered, hollowed-out look
cannot necessarily be fixed with cosmetic surgery. Prevention is best.

fact, recent research on the subject indicates that smokers have twice
the risk of losing their teeth than non-smokers. According to
researchers, after accounting for variables such as age, gender, and
coffee/alcohol consumption, smokers were found to lose significantly
more teeth than their non-smoking counterparts. Researchers speculate
that the reason for this is that smoking damages the bone that holds
teeth in place. As a result, smokers not only develop wrinkles and
creases around their mouths, they may also suffer a vertical collapse
of their lower faces due to lost teeth.

reshaping (or “enamel recontouring”) can alter the shape, length,
contour, or position of a tooth and its relationship to adjacent teeth.
The technique can also be used to reshape chipped, overlapped, or

can be accomplished without removing substantial amounts of tooth
structure. Only one office visit and no anesthesia are required.

Each year,
dentists treat millions of cavities that result from the enamel-eating acids
emitted by sugar-eating bacteria missed by flossing and brushing. While
drilling out tooth decay and filling cavities with amalgam or composite
fillings has proven extremely effective in preserving the remaining healthy
tooth structure, patients often wish they could somehow magically eliminate
any decay and simply regenerate the tooth back to its previous healthy state.
That wish may someday be a reality because scientists have identified a
peptide known as MSH (melanocyte-stimulating hormone) that encourages cells
within decayed teeth to regenerate in about one month. When introduced into
the teeth in a gel or film in the laboratory, cavities disappeared and were
replaced by healthy tooth structure.

of a tooth becoming abscessed (due to infection) or traumatized (due to
injury), it is often necessary to perform root canal therapy to remove the
dead or damaged root pulp. The goal of the procedure is to save the crown
(white outer portion of the tooth). The pulp is the area inside the crown that
extends down into the gums with nerves and blood vessels. The dentist can
check to see if the tooth is alive by testing whether the patient can feel
cold on the tooth or reacts to an electrical stimulation on the tooth. The
procedure involves removing the pulp from the non-vital tooth and filling and
sealing the area and securing the crown.

awareness, not causing alarm. What many people do not know is that
Streptococcus mutans is a bacterium that can be transferred by saliva when
utensils are shared and even when parents kiss their babies. It’s important to
note that only parents and caregivers with active tooth decay can spread
Streptococcus mutans bacteria when saliva is transferred. Beyond that, tooth
decay develops as a result of a combination of factors, including genetics,
oral hygiene, and exposure to sugary liquids. Good oral health on the part of
parents helps minimize the bacteria-transfer factor.
P.S. Babies
are particularly vulnerable to tooth decay during the time of tooth eruption,
when their tooth enamel is relatively soft.

that dentists insist that their patients floss (as well as brush) their teeth
is because
flossing does

are advised
to use a flat,
wide dental tape. If teeth are closely spaced, thin floss is best, especially

with a
stiff end that
does the job equally well.

of smokeless tobacco should understand that they face much the same risk
of developing

that the use of smokeless tobacco “is not a safe substitute for smoking
cigarettes. It

nicotine
addiction and dependence.” Both chewing tobacco and snuff (shredded tobacco in

parents who make regular visits to the dentist are more likely
to see the
dentist themselves. According to research involving over 6,000
children
between the ages of two and seventeen years, researchers found
that about 86%
of children whose parents had a dental visit during the preceding
year had
dental exams compared with 63% of children whose parents had not
scheduled
exams for themselves. With tooth decay as one of the most chronic
diseases in
the United States, parents are urged to focus on their families’
oral health
needs. Treatment delayed is likely to be a good deal costlier,
in terms of
dollars as well as outcome, than immediate or preventive
treatment of
cavities and gum disease.

While x-rays
and probing instruments are important tools for identifying
tooth decay, a
laser that detects cavities can uncover even the smallest
amounts of
decay in the hardest-to-reach places. The DIAGNodent utilizes
a laser-probe
hand piece that emits pulsed light of a known wavelength
on tooth
surfaces. Once a baseline measurement of the patient’s healthy
tooth surfaces
is established, further examination of potentially
problematic
areas creates changing levels of fluorescent light,
which the
unit’s internal processor interprets. An audible signal warns
of an
indication of caries (tooth decay). The laser unit can also be
used to keep
track of early symptoms of tooth decay, which translates
into earlier
treatment and better tooth preservation.

PLASMA
JETS INSTEAD OF DRILLS?

The future of
dentistry may be more akin to science fiction than patients
might ever
have believed. In fact, it seems that new research involving
streams of
bacteria-killing plasma may replace today’s drills in the near
future. Plasma
is formed when electrons are emitted by energized gases.
In this case,
researchers used a relatively cool beam of purple plasma
to eliminate
oral bacteria in dental cavities. The plasma beam removed
dentin (the
material beneath the enamel that comprises the majority of
the tooth)
more selectively than a conventional drill. While this research
is
preliminary, researchers believe that plasma jets can be ready for
removing tooth
decay in as few as three to five years.
P.S. It isn’t
the heat of the plasma that is thought to kill
cavity-causing
bacteria; it is the highly reactive molecules produced by
charged oxygen
molecules surrounding the plasma jet that get the job done.

OUT OF THE
ROTATION

There are
instances in which a permanent tooth will be rotated on its axis so
that it isn’t
aligned properly with either adjacent or opposing teeth. As a
result of its
twisted orientation, the rotated tooth creates a space on both
of its sides
where it should otherwise stand shoulder-to-shoulder with the
teeth next to
it. In cases where the rotation is not too extreme, the dentist
can augment
the rotated tooth with color-matching bonding material. This
composite
restoration is undertaken in an incremental manner to slowly fill in
gaps and build
up the tooth in areas where it is needed. All the while,
care must be
taken to ensure correct tooth dimension and attain the desired
aesthetic
result.

PREGNANCY
AND GUM DISEASE

Dentists have
long cautioned women about “pregnancy gingivitis,” which is caused by hormonal
changes and
affects more than half of all pregnant women. Usually, the gum inflammation
associated
with pregnancy-related gum disease occurs between the second and eighth months
of pregnancy,
and manifests itself in gums that look red and bleed somewhat during brushing.
Without
conscientious attention to good oral-hygiene habits and professional dental
care,
this mild form
of gum disease (gingivitis) can lead to a more serious form of gum disease
(periodontitis). If left untreated, gum disease poses risks to babies’ health
such as
premature
birth, low birth weight, and even full-term stillborn birth. Pregnant women
have more
reason than their own oral health to seek regular dental care.
P.S. During
pregnancy, increased levels of progesterone may encourage the growth
of certain
gingivitis-causing bacteria, render gum tissue more sensitive to plaque,
and exaggerate
the body's response to the toxins that result from plaque.

CORROSIVE
EFFECT OF EATING DISORDER

The eating
disorder known as bulimia poses a problem for the teeth
that may be
overlooked. Repeated purging of stomach acids can cause
significant
loss of tooth enamel. In one such case, a person with a
history of
bulimia began treatment to correct a closed bite, characterized
by decreased
vertical tooth dimension and an abnormal overbite in which
the lower jaw
protrudes. Due to stomach acid-induced enamel loss, the
patient’s
incisors lost one-third to one-half of their original length.
The closed
bite was caused by over-eruption of the lower incisors into
the space
created by eroded teeth. Fortunately, treatment of the eating
problem led to
aesthetic correction involving porcelain veneer crowns
that could be
used to restore the damaged teeth.

ORAL
CANCER CHECK

In light of
the fact that more than 34,000 Americans are diagnosed with oral
cancer
annually, it is a good thing that dentists regularly perform exams to
detect cancers
of the mouth. Such exams are very important since early
detection
leads to treatment, which leads to better outcomes. When oral
cancer is
detected in its earliest stages, the five-year survival rate is
80 percent to
90 percent. However, if oral cancer is left undetected for
as little as a
few months, the survival rate after five years drops to
55 percent. As
with any disease, the best approach is prevention. All
forms of
tobacco products damage the DNA of cells in the mouth. Other
risk factors
include heavy alcohol consumption.
P.S. Alcohol
consumption also compounds the risk of developing oral
cancer posed
by tobacco because a compound in alcohol (aldehyde)
increases the
absorption of cancer-causing substances in tobacco.

WISE
CHOICE?

“Wisdom
teeth,” which are the last teeth to erupt in the mouth, are somewhat
controversial.
While some people retain these teeth their whole lives, others choose
to have them
extracted, sometimes before they emerge from beneath the surface
of the gums.
The decision to remove wisdom teeth or leave them alone is one
that should be
made with the dentist. During an evaluation between ages 16
and 19 years
of age, the dentist may recommend extraction if a wisdom tooth
is crowding
other teeth, causes pain, or has become impacted (stuck below the surface).
Infection is
another good reason to have a wisdom tooth extracted. Impacted
wisdom teeth
are often extracted by age 20 to prevent potential future problems.

Gum disease
results from accumulations of bacteria-laden plaque and tartar
that cause
infections in pockets in the collar of gum tissue that
surrounds the
teeth. Most of these bacteria are anaerobic, meaning that
they can
thrive without oxygen beneath the gumline. In the early stages of
gum disease
(gingivitis), the infections may be adequately treated with
professional
cleaning and conscientious home care. However, if the disease
is allowed to
progress to a more advanced stage (periodontitis), more
aggressive
remedial measures may be called for. Scaling and root planing
is the
procedure by which surfaces of the tooth roots are planed with
scraping
instruments until tartar is removed and tooth surfaces are
sufficiently
smooth for gum tissue to adhere to them.

The reason
that so many patients regard root canal (endodontic) treatment as painful is
that
they confuse
the condition that makes treatment necessary with the treatment itself. If
dental decay
is not dealt with promptly, bacteria may infect tooth pulp. This inflammation
or infection
can, in turn, lead to an abscess if left untreated. Symptoms include aching
or throbbing
pain in the tooth. At this point, endodontic treatment is indicated.
This
procedure, which relieves painful symptoms, involves removal of the nerve and
vascular
tissue (pulp) from the root and pulp chamber, as well as any associated
decayed tooth
structure. After the inside of the tooth is carefully shaped and cleaned,
it is filled,
sealed, and ready for a crown.

SUFFICIENT
BONE FOR AN IMPLANT?

One of the
primary requirements for successful dental implant surgery is a sufficient
amount of
bone to
accommodate the titanium implant. In most cases, bone will usually fill in the
socket
left by an
extraction within three months. However, if the walls of the socket are
particularly
thin, there
may not be enough bone to accept the implant. This is particularly true of the
upper
and lower
front teeth, where there may be only a thin ridge of bone (unlike the wide
area of
jawbone at the
back of the mouth, where the molars reside). If so, a bone graft can be placed
in the socket
to help the body fill the hole left by the tooth root with bone.
P.S. A bone
graft may also be recommended for patients who lost teeth many years ago and
do not have enough bone left for an implant.

DRYING
EFFECTS

Patients with
symptoms of “dry mouth” should notify the dentist. Not only can reduced saliva
production
lead to difficulty speaking, swallowing, persistent bad breath, and
chapped/cracked
lips, it can
also potentially contribute to cavities, gingivitis, and other dental
problems.
Once the
dentist is made aware of dry mouth, there are ways to measure saliva
production
and evaluate
the problem. Because dry mouth is a side effect of various drugs, a proper
evaluation
also entails a patient’s detailed description of any medications that he or
she
is taking. If
dry mouth only occurs at night, it may be a symptom of a nasal obstruction
that
may require
further medical evaluation. Dry mouth is also a symptom of some auto-immune
disorders.
P.S. It is
normal for saliva production to decline by as much as 40 percent as we grow
older.

single office
visit without impressions, temporaries, or second office visits. This is

special camera to take a picture of the damaged tooth, the image of which is

GOOD ORAL
HEALTH PROMOTES CLEAR THINKING

There is
already research that links gum disease with heart disease, stroke, diabetes,
and
Alzheimer’s disease. Now, a recent study suggests that gum disease may also be
associated
with poorer thinking skills. Researchers found that adults aged 60 and
older with the
highest levels of the gum disease-causing pathogen Porphyromonas
gingivalis
were three times more likely to experience trouble recalling a three-word
sequence over
time than those with lower levels of the pathogen. Adults with the highest
levels of the
pathogen were also two times more likely to fail three-digit reverse
subtraction
tests. It is thought that gum disease influences brain function through
several
mechanisms, including causing body-wide inflammation, a risk factor for loss
of mental function.
P.S. As
scientists slowly collect evidence that links oral health with brain
function and
overall health, patients have more reason than ever to brush and floss and

Even before
infants’ baby teeth erupt, parents can take steps to ensure their children’s
oral
health.
Beginning with using a damp gauze pad to gently wipe baby’s gums, parents can
remove
residual milk,
formula, and fruit juices from the mouth. These liquids contain sugars that
bacteria feed
on and produce acids that attack teeth. For this reason, a baby should not be
allowed to
fall asleep with a bottle of juice or formula in his or her mouth. Parents
should
start brushing
their children’s teeth with a soft-bristled toothbrush and a pea-sized amount
of toothpaste
as soon as the first tooth appears. Flossing to prevent cavities between teeth
should start
when two teeth have erupted next to each other.

CLEANING
DENTURES

Dentures may
be “false teeth,” but they still must be cleaned the same way that natural
teeth are. As
with real teeth, dentures are subject to buildups of sticky plaque, which
poses the
threat of infection and inflammation for gums. With this in mind, denture
wearers are
strongly urged to either brush their prosthetic teeth or soak them in cleaning
solution.
Arguably, the best cleaning method is brushing with toothpaste. However, for
those who
experience difficulty brushing their dentures effectively, soaking can remove
as much plaque
as brushing. For a short soak of about 15 minutes, enzymatic and
effervescent
solutions are probably equally effective at removing plaque. For
longer soaks
(up to eight hours), enzyme denture-cleaning solutions may be preferable.
P.S.
Ultrasonic cleaning is about as effective as brushing when it comes to
cleaning dentures.

STICKING
TO YOUR GUMS

It is
estimated that four out of five U.S. adults have some form of gum disease,
which ranges
from simple gum inflammation (gingivitis) to serious disease
(periodontitis) that has the potential to damage bone and loosen teeth.
For these
reasons alone, adults have good reason to stop gum disease before
it has a
chance to cause severe damage. Without conscientious attention to
brushing and
flossing at home and regular professional cleanings, gums may
become red and
swollen, and bleed easily. In addition, the gums may form
pockets around
teeth that harbor bacteria-laden plaque. This colorless,
sticky film
can solidify as tartar, which has been likened to the barnacles
on a ship and
is similarly difficult to remove.

SEEING
RED!

A dentist may
be likely to provide a natural redhead with a bit more
attention than
blondes and brunettes might receive. The reason behind
this special
treatment rests with the fact that redheads seem to have a
genetic
disposition to be nervous about visiting the dentist. In fact,
research in
recent years shows that redheads tend to be less responsive
to local
pain-blockers and may require larger doses of Novocain
to get the
same amount of relief that a blonde or brunette would
get with
regular doses. More recent research shows that the red hair
that results
from variations in the melanocortin-1 receptor (MC1R) gene,
which helps
determine pigmentation, may also play a role in processing pain, anxiety, and
fear.

INCORRECTLY PEGGED FROM THE BEGINNING

So-called “peg
laterals” are small, cone-shaped teeth that develop on
either side on
the top two front teeth. These undersized teeth appear
to be “baby
teeth” surrounded by larger permanent teeth. As a result,
the smile
looks imbalanced. Fortunately, the dentist can restore peg
laterals to
the size of normally developed incisors by building them up
with composite
material. This tooth-colored resin restoration requires
no anesthetic
and can be accomplished in one visit. After the dentist
acid-etches
the tooth’s enamel so it can properly accept the resin,
layer upon
layer of resin is applied to the tooth. After the
composite
material cures properly, the augmented tooth is shaped
and polished.
The result compares favorably with surrounding natural teeth.

WHAT
SENIOR PATIENTS CAN EXPECT

can expect some age-related changes in their teeth and gums,
certain
changes should not be overlooked. For example, it’s natural for
teeth to
darken and show signs of wear as we age. Aging gums are also likely
to recede,
which has given rise to the phrase “long in the tooth.” As a

dental cavities closer to the root of the tooth are more likely to
occur.
However, neither tooth decay nor tooth loss should be accepted as a
normal part of
the aging process. Bleeding gums is a sign of gum disease,
which is the

addressed.

CLOSING
THE GAP

Gaps between
the upper two front teeth may appear
for the simple
reason that there is more room than necessary
to accommodate
all of the teeth. Otherwise, it may be that
the muscle
attachment (frenum) that is found between the upper
lip and the
upper dental arch (in the area of the two front teeth)
is too large
or fibrous. If so, the front teeth may splay. A simple
surgical
procedure may be all that is necessary in this case to relieve
the pressure
exerted by the frenum. After that, the gap between the two
front teeth
may be closed using a bonding procedure that builds up tooth
material on
both sides of the space.

observing high levels of dental erosion among patients, which is

symptoms are
due to loss of the teeth’s protective enamel coating that

away under an assault from the acids found in a variety
of beverages.
The primary culprit is phosphoric acid, a primary flavoring
agent in soft
drinks, especially colas. In fact, phosphoric acid is the
substance that
dentists use to etch the surfaces of teeth that are about
to receive the
bonding agents used for patching and repairs. Sugar found
in many soft
drinks also plays a role in tooth erosion by feeding the
bacteria in
plaque that eat away at teeth.
P.S. To
counter the problem of acid-induced dental erosion, dentists
suggest
limiting soft drink, sport drink, and fruit juice intake, along
with increased
use of fluoride, which builds up tooth enamel.

DO MEN
BRUSH AS OFTEN AS WOMEN?

American Dental Association recently conducted a national survey,
they found
that men and women differ when it comes to taking care
of their teeth
and gums. According to the nationally representative
survey of
1,000 Americans ages 18 and older, 86 percent of women brush their
teeth twice or
more a day while only 66 percent of men make the same effort
to keep their
teeth clean. As far as flossing is concerned, there is room for
improvement
for both genders. Slightly less than half of those surveyed (49%)
indicated that
they floss their teeth once a day or more. One out of
three people
surveyed think that bleeding gums after brushing their teeth is normal—but it
is not.

four months on
average while men retain theirs an average of five months.

new treatment may provide dental patients with another
option for
treating receding gums. The traditional treatment for
this problem
involves surgically grafting tissue from the roof
of the
patient’s mouth and transplanting it onto the patient’s
receding gums.
The alternate procedure, known as “guided tissue
regeneration”
(GTR), begins with drawing blood from the patient,
from which
blood cells known as platelets are retrieved.
The value of
these platelets to dental researchers rests with
the fact that
they are rich in proteins called “growth factors,”
which promote
tissue repair and wound healing. Researchers found
that soaking
a collagen membrane with the platelets, then suturing
it over the
exposed tooth root, provided results comparable to the grafting method.

SUBSTITUTE AS CAVITY FIGHTER

xylitol has already been proven to have decay-fighting

ingredient in chewing gum among school-age children
with permanent
teeth. More recently, researchers have found that
much younger
children, who received eight grams of xylitol daily

also experienced a significant cavity-fighting
benefit. In
fact, the baby teeth of children between the ages of 9
and 15 months
who received the daily dose of xylitol experienced
up to a 70
percent reduction in tooth decay. However, researchers
were quick to
point out that this is not to be construed as a
silver-bullet
approach, as fighting tooth decay should be a multi-pronged effort.

GOING

comprehensive oral exam involves both an examination of the teeth and a
close
inspection of the soft tissues of the mouth. One condition the dentist
or hygienist
will be looking for are “leukoplakia.” This generic term applies
to any type of
white patch that occurs inside the mouth that cannot be
easily scraped
off. In some cases, these patches represent calluses
or some other
benign condition. In others, leukoplakia are associated
with smoking
and regular, heavy alcohol use. If so, they are considered to
be a
precancerous condition. Smokers and heavy drinkers may have tongues with
white or brown
coatings that are common manifestations of leukoplakia. Most
cases are not
serious, but others could be life-threatening if left untreated.

Leukoplakia is usually harmless, and lesions generally clear
in a few weeks
to a month after the source of irritation is removed.

This involves removal of the nerve and vascular tissue
(pulp) from
the root and pulp chamber, as well as any associated

extraction.
After the inside of the tooth is carefully shaped and
cleaned, it is
filled and sealed, awaiting a crown. Preserving
the tooth root
in this fashion helps avert the bone loss that
often
accompanies loss of the tooth root.

regular checkup, the hygienist can often tell whether

right-handed or left-handed simply by identifying the

brushed more
rigorously than others. In addition, most people

the outer surfaces of their teeth more completely
than the inner
(lingual) sides. As a result, the inside surfaces
of the lower
front teeth are the ones that accumulate the most
plaque and
tartar. The gums in these areas are more likely to be
susceptible to

the insides of the lower front teeth be brushed first.

bacteria in the mouths of heart attack victims, the most common indicator
of heart

As public

guidelines set forth by the Occupational Safety and Health
Administration, which ensure protection from infectious diseases.
These measures
include steam autoclaving all instruments and hand pieces
after every
use. Disposable items are used whenever appropriate.
Dentists and
staff wear protective eyeglasses and disposable gloves during
treatment. All
areas that are touched during the procedures are covered
with a plastic
wrap and changed after each patient to eliminate cross
contamination.
All surfaces, chairs, lights, and units are thoroughly
disinfected
before and after every patient’s use. Attention to these and
other measures
ensures our patients’ (and our staff’s) health and safety.

OBESITY
INCREASES PERIODONTAL-DISEASE RISK

In its mild form, gingivitis
(gum disease) may only lead to bleeding gums. However, if not
treated, gingivitis can
progress to a more severe stage, periodontitis, in which bacteria-laden
plaque provokes an
inflammatory response in gums. To avoid the potential of infection and
tooth loss, it is important to
identify causes of gum disease, as well as to diagnose and
treat it. With this in mind,
researchers have recently found that obese adults are at higher
risk of gingivitis than people
of normal weight. In fact, over the course of the 16-year study,
obesity was found to be
associated with a 29 percent increase of periodontal disease.
The common denominator is
thought to be inflammation.
P.S. In addition to the
obesity link with gum disease in the study mentioned above, a
waist measurement of 40 inches
or more in men was linked to a higher risk of heart disease.

ANTIBIOTICS BEFORE DENTAL PROCEDURES?

Patients with
hip prostheses may have heard that it is advisable to
take
antibiotics prior to undergoing tooth cleaning, scaling, and
other
procedures to address the potential release of bacteria into
the
bloodstream caused by these treatments. For people with replacement
joints, the
concern is that bacteria in the bloodstream (“bacteremia”)
could cause an
infection by attaching to the artificial joint or
surrounding
tissues. However, such infections are quite rare. In fact,
the American
Dental Association recommends against the use of
antibiotics
prior to dental procedures with the sole intention of
preventing
infections in artificial joints. Exceptions to this
recommendation
involve patients who are very susceptible to infection or
are having a
procedure that carries a higher risk of bacteremia.
P.S. Pre-
treatment antibiotics may be indicated for immune-compromised
patients and
those who have inflammatory arthritis, type 1 diabetes, or hemophilia.

Interestingly, the American Dental
Association notes that teeth were “designed”
to last only about 30 years, which
means they may need a little help in keeping
up appearances. Aside from the usual
problems of decay and discoloration,
older adults face other potential
problems that can undercut the youthfulness
of their smiles. For instance, the
incisors (or canines) wear down as the
years pass, leaving less enamel
exposed below the upper lip when a person smiles.
Moreover, teeth drift toward the
center of the mouth as the bone and tissues
that hold them in place weaken with
age. As a result, teeth can appear crowded
and unevenly distributed. Fortunately,
there are cosmetic procedures available
that can help restore a smile’s
youthfulness.
